The trigeminocardiac reflex (TCR) is defined as the sudden onset of parasympathetic dysrhythmia, sympathetic hypotension, apnoea or gastric hyper-motility during mechanical/thermal stimulation of any of the sensory branches of the trigeminal nerve. The risk factors that are already known for increasing the prevalence of the TCR include anatomical location, hypercapnia, hypoxemia, light general anaesthesia, age (more pronounced in children), the nature of the provoking stimulus (stimulus strength and duration) and different drugs. Already different potential confounders are also identified. This discussion about risk factors has its importance because of the substantial consequences for functional outcome after intraoperative TCR occurrence. But there remains still a substantial lack of thorough understanding of the TCR, the current treatment options for patients with TCR include a mostly empirical approach: (i) risk factor identification and modification; (ii) prophylactic measures of vital signs and (iii) administration of vagolytic agents or sympathomimetics. In this context, we have now created different thinking models so that we can preoperatively plan a skull base surgery procedure safely in relation to a potential occurrence of the TCR episodes. This chapter provides an overview of this unique reflex that presents a unique interaction between heart and brain. In addition, this also illustrates the mechanism of various cardiac rhythm changes related to the TCR.

U2022 Background : Trigeminocardiac reflex occurs after manipulation around any branches of the trigeminal nerve. Oculocardiac reflex has frequently been reported. However, there have been only rare reports of such cases attributed to stimulation of division III. We report a case of trigeminocardiac reflex in open reduction for TMJ dislocation.u2022 Case report : A 74-year-old female presented for TMJ dislocation. Manual reduction was failed because of severe lateral pterigoid muscle contracture. Open reduction under general anesthesia was decided. During surgical manipulation of right mandibular angle, severe bradycardia(HR=20bpm) occurred. Immediately 0.5 mg atropine was administered intravenously, and the surgical manipulation was stopped. After 60 seconds, heart rate normalized. Then, during surgery, severe bradycardia occurred one more time. It disappeared spontaneously as soon as surgical manipulation was stopped. The surgery was completed uneventfully.u2022 Discussion : Trigeminocardiac reflex due to stimulation of division III is very rare. However, given the possibility of profound bradycardia, asystole, or even death when evoked, it is important to be aware of the trigeminocardiac reufb02ex during surgical stimulation of the mandibular divisions of the trigeminal nerve, in particular surgical stimulation of TMJ.u2022 References : William D. Abstract : Abstract: Hemodynamic alterations are observed in various neurosurgical procedures and commonly related to different neurogenic mechanisms. However, anesthetic influences on causation of these perturbations or management are rarely investigated and therefore our present knowledge is still limited. In this case of 43-old Caucasian male, propofol boluses aborted the trigeminal cardiac reflex (TCR) induced severe bradycardia during dural manipulation. There is a correlation of severity of bradycardia and slightness of anesthesia. In the light of the larger distribution of the TCR all over the world, we see more and more aborted TCR, as seen in the present case; then the neuro-anesthesists more and more recognize the TCR at its very onset. A surrogate model for the daily use is present to underline the clinical needs. We have therefore developed, for the first time, a surrogate model that helps in daily practice to recognize and prevent TCR episodes.
