INTRACAVERNOUS INTERNAL CAROTID ARTERY RUPTURE DURING PITUITARY ADENOMA TRANSSPHENOIDAL SURGERY: A CASE REPORTCuu00f1at TM, Su00e1nchez D, Maldonado F, Tercero FJ, Valero R, Ensenyat J BACKGROUND Transsphenoidal surgery does not lack risks. Intraoperative bleeding due to injury of the cavernous portion of the internal carotid artery (ICA) is an infrequent complication whose outcome can be fatal.CASE REPORT A 47-year-old man with a pituitary tumour involving the right intracavernous ICA, secondary hydrocephalus and ventricular-peritoneal shunt underwent transsphenoidal tumor resection. During surgery, rupture of the ICA with hemorrhagic shock occurred (bled 2L) requiring hemodynamic stabilization (vasoactive drugs, fluids, tranexamic acid, massive transfusion); surgical attitude included intranasal hemorrhage control and proximal carotid control (vessel loop at the cervical level). After his stabilization, patient was transferred to DIVAS where carotid flow was verified and a stent was installed in the bleeding area. In the first 24 hours, stent thrombosis and stroke of the border territory between the anterior cerebral artery (ACA) and the right middle cerebral artery (MCA) appeared. Since neurologic sympthoms were related to cerebral perfusion pressure, an extra-intracranial bypass of the superficial temporal artery to the distal branch of MCA was performed to improve blood flow. After an initially functioning bypass, absence of flow despite 3 attempts of anastomosis was evidenced by indocyanine green. Three days later, right cortical acute ischemic lesions were observed in MCA area, border territory between ACA and MCA and in the right internal capsule. Despite adequate compensation through the anterior and posterior communicating arteries, a right hemispheric syndrome occurred. Fortunately, neurological signs progressed favorably until complete remission in 17 days.DISCUSSION Risk factors for intracavernous ICA injury during transsphenoidal surgery include bulky and invasive adenomas, previous radiotherapy and medical treatment of the adenoma. Its complications may be fatal, so transsphenoidal pituitary surgery should be performed in centers with endovascular therapy.CONCLUSSIONS Patients with risk of carotid injury should be identified. There must be previous communication between neurosurgery, neuroradiology and anesthesiology due to potential lntracavernous ICA damage. If it happens, a protocolized approach allows minimization of consequences. Cerebral border territories may not be well perfused despite a functional Willis polygon if it is not accompanied by adequate carotid flow.

It is only recently that the use of the endoscope as the sole visualizing tool has been introduced in transsphenoidal pituitary surgery with its favorable related implications and minimal operative trauma. Of course, microscopic and endoscopic anatomy are basically the same, but the optical distorsion of endoscopic images is quite substantial compared to microscopic depictions. An endoscope lens produces images with maximal magnification at its center and severe contraction at its periphery. Nearer images are disproportionally enlarged and remote images are falsely miniaturized.
