Radioactive iodines are produced during the operation of nuclear power plants and during the detonation of nuclear weapons. In the event of a radiation incident, radioiodine is one of the contaminants that could be released into the environment. Exposure to radioiodine can lead to radiation injury to the thyroid, including thyroid cancer. Radiation to the thyroid from radioiodine can be limited by taking a nonradioactive iodine (stable iodine) such as potassium iodide. This book assesses strategies for the distribution and administration of potassium iodide (KI) in the event of a nuclear incident. The report says that potassium iodide pills should be available to everyone age 40 or youngerâ€"especially children and pregnant and lactating womenâ€"living near a nuclear power plant. States and municipalities should decide how to stockpile, distribute, and administer potassium iodide tablets, and federal agencies should keep a backup supply of tablets and be prepared to distribute them to affected areas.

As a sequela to the large release of 131I from the accident at the Chernobyl Nuclear Power Plant, an expected late effect is thyroid cancer, especially in children. In anticipation of this problem, hundreds of thousands of measurements of thyroid glands were made with survey meters. Much attention was also focused on measuring the deposition density of 137Cs. The expectation was that the latter measurement could be a good surrogate for the deposition density of 131I, so that ecological models could be used to reconstruct thyroid doses in locations where no direct measurements of thyroid activity were made. However, this assumption has been seriously questioned, and there is interest in a more suitable surrogate that can still be measured even nine years or more after the accident. The purpose of this paper is to discuss the reconstruction of thyroid doses for a case-control study of childhood-thyroid cancer that has just been concluded, to discuss the reconstruction of thyroid doses for a current cohort study of childhood-thyroid cancer, and to discuss the use of 129I as a surrogate for the deposition density of 131I.

This report describes dose-response models intended to be used in estimating the radiological health effects of nuclear power plant accidents. Models of early and continuing effects, cancers and thyroid nodules, and genetic effects are provided. Weibull dose-response functions are recommended for evaluating the risks of early and continuing health effects. Three potentially lethal early effects -- the hematopoietic, pulmonary, and gastrointestinal syndromes -- are considered. In addition, models are included for assessing the risks of several nonlethal early and continuing effects -- including prodromal vomiting and diarrhea, hypothyroidism and radiation thyroiditis, skin burns, reproductive effects, and pregnancy losses. Linear and linear-quadratic models are recommended for estimating cancer risks. Parameters are given for analyzing the risks of seven types of cancer in adults -- leukemia, bone, lung, breast, gastrointestinal, thyroid, and other.'' The category, other'' cancers, is intended to reflect the combined risks of multiple myeloma, lymphoma, and cancers of the bladder, kidney, brain, ovary, uterus and cervix. Models of childhood cancers due to in utero exposure are also developed. For most cancers, both incidence and mortality are addressed. Located in the Ukraine, the Chernobyl power plant was undergoing experiments in the early morning hours of April 26, 1986 when it suffered a series of explosions in one of its nuclear reactors, killing over 30 people at the plant and spread radioactive fallout across a wide swath of the Soviet Union.
