Nitric oxide (NO) is a bioactive gas that has multiple roles in innate and adaptive immune responses and dysregulated expression of its production by inducible nitric oxide synthase (iNOS) is implicated in the pathogenesis of various inflammatory diseases and cancer. The mechanisms by which the expression of iNOS is regulated in human cells is incompletely understood. I show that NO positively regulates iNOS expression through a positive feedback mechanism that involves S-nitrosylation and activation of the small Ras GTPase, which in turn activates downstream PI3K/Akt and mTOR pathways. This feedback mechanism acts in a post-translational manner to increase iNOS protein levels by reducing its ubiquitination and proteasomal degradation. In addition to examining the feedback regulation of iNOS protein expression, I also studied how gene expression of this enzyme is controlled. I focused on the hypoxia inducible factor (HIF) because of the central role of this transcription factor in controlling many aspects of cell biology. The human iNOS gene promoter has three predicted HIF binding sites and mutation of one of these sites at -4.9 kb reduced the induction of iNOS gene promoter activity by cytokines. Further, HIF-1a gene editing by CRISPR/Cas9 eliminated HIF activity and reduced the induction of iNOS mRNA and protein expression by cytokines. Altogether, I discovered two mechanisms whereby iNOS expression is positively regulated at the protein and gene expression levels in an inflammatory setting. The role of NO and HIF in augmenting iNOS expression may provide insight into new therapeutic strategies for inflammatory diseases and cancer.

Nitroglycerin and other organic nitrates have been used for over a century in the treatment of angina pectoris. Millions of patients, throughout the world, have placed nitroglycerin tablets under the tongue and have experienced rapid and dramatic relief from the chest pain that frequently occurs as a manifestation of disease of the coronary arteries. The empirical observation of the safe use of nitrates for tile alleviation of the symptoms of angina have led to their widespread medical acceptance. Similarly, considerable effort has been expended in understanding the mechanism of action of these agents directly on vascular smooth muscle and on cardiac work and performance. Finally, there is a more substantive understanding of the physiology of the coronary circulation as well as the" pathophysiologic manifestations of myocardial disease.

Inflammation has been described as the basis of many pathologies of human disease. When one considers the updated signs of inflammation, they would be vasodilation, cell migration, and, in the case of chronic inflam- tion, cell proliferation, often with an underlying autoimmune basis. Gen- ally, inflammation may be divided into acute, chronic, and autoimmune, - though the editors believe that most, if not all, chronic states are often the result of an autoimmune response to an endogenous antigen. Thus, a proper understanding of the inflammatory basis may provide clues to new therap- tic targets not only in classical inflammatory diseases, but atherosclerosis, cancer, and ischemic heart disease as well. The lack of advances in classical inflammatory diseases, such as rh- matoid arthritis, may in part arise from a failure to classify the disease into different forms. That different forms exist is exemplified in patients with d- fering responses to existing antiinflammatory drugs, ranging from nonresponders to very positive responders for a particular nonsteroidal an- inflammatory drug (NSAID). Though researchers have progressively unr- eled the mechanisms, the story is far from complete. It should also be noted that the inflammatory response is part of the innate immune response, or to use John Hunter’s words in 1795, “inflammation is a salutary response.” That may be applied in particular to the defensive response to invading micro- ganisms.
