Book Description :

Millions of procedures, surgical and non-surgical, are performed around the world each year. Management of perioperative pain is of great importance to patients, and a critical management issue for physicians and other health professionals who provide perioperative care. Healthcare facilities and national accrediting organizations have established standards surrounding proper management of perioperative pain. There is now an increased burden on every hospital and training program to ensure that healthcare providers understand the essentials of pain management and are able to recognize and treat pain in a timely fashion. Poorly controlled pain leads to patient dissatisfaction and contributes to increased morbidity and mortality, such as myocardial infarction, pneumonia, and emotional effects. Proper management, including appropriate pharmacologic management and regional analgesic techniques, can improve function and shorten length of hospital stay. Patients who are undergoing procedures under sedation or anesthesia need to receive adequate pain relief with drugs or regional anesthetic techniques. In the recovery room following the procedure, the staff taking care of the patient needs to be properly trained to diagnose and treat post-procedural pain. Part of the Oxford American Pain Library, this concise, evidence-based clinical guide serves as a tool for every clinician who wishes to understand the basic mechanisms, pharmacology, invasive and noninvasive treatment modalities, guidelines and development of pain management protocols. The authors address new technologies, chronic pain issues, running an acute pain service, opioid and non-opioid pharmacology (including newly approved drugs), epidural and other regional anesthesia, and special populations such as pediatric patients, the elderly, and patients with a co-existing disease.

Patientsu2019 opinions on acupuncture treatment for perioperative complications: a Chinese surveyFen Wang1, Man Zheng1, Juan Zhu1, Fang-bing Ji1, Wen-zhong Wu2, Zhen Zheng31 Affiliated Hospital of Nanjing University of Traditional Chinese Medicine, Dept of Anaesthesiology, Nanjing, China;2 Affiliated Hospital of Nanjing University of Traditional Chinese Medicine, Dept of Acupuncture and Rehabilitation, Nanjing, China;3 School of Health and Biomedical Sciences World Health Organization Collaborating Centre for Traditional Medicine RMIT University, Discipline of Chinese Medicine, Bundoora, Australia.ABSTRACTBackground and Goal of study: Previous studies have shown that acupuncture has a broad range of effects on perioperative complications. This survey was aimed to assess the surgical patientsu2019 opinions on the application of acupuncture for perioperative complications and to explore the possible factors that influenced their opinions. Methods: A 20-item questionnaire was developed and conducted among adult surgical inpatients in the Affiliated Hospital of Nanjing University of Traditional Chinese Medicine in China over a period of eight weeks. Patients who couldnu2019t understand the questionnaire were excluded. The questionnaire included demographic characteristics (gender, age, education, and occupation), patientsu2019 knowledge about acupuncture and patientsu2019 opinions on acupuncture treatment for perioperative complications. We obtained verbal consent from patients before giving them the questionnaire to complete. Descriptive tests were used to present data of demographics, patientsu2019 knowledge and opinions. Chi-square was used to explore the factors that might affect patientsu2019 opinions on using acupuncture for postoperative nausea and vomiting (PONV).Results and Discussion: 176 patients completed the questionnaires with a response rate of 98.9%. Less than half of them were willing to use acupuncture for PONV (n=70, 39.8%), postoperative pain (n=79, 44.9%), preoperative anxiety and insomnia (n=65, 36.9%), postoperative gastrointestinal dysfunction (n=76, 43.2%), and postoperative urinary retention (n=81, 46.0%). Patients who were professionals or office workers (chi square=26.181, p=0.001) or with better education (chi square=14.583, p=0.024) were more willing to use acupuncture for PONV while age or gender didnu2019t affect their opinions. However, those who had history of PONV were less willing to use acupuncture for PONV (chi square=10.664, p=0.005). Conclusions: Overall, the willingness of the surveyed Chinese patients to use acupuncture for perioperative complications was moderate (about 40%). Having higher educational levels or a professional occupation had a positive impact on their opinions on acupuncture treatment for PONV whereas PONV history had a negative impact. This survey was only taken among surgical patients in one hospital, further multi-center studies are needed to understand the opinions of patient populations with different backgrounds and different health conditions.

Book Description :

Cancer is one of the leading killers in the world and the incidence is increasing, but most cancer patients and cancer survivors suffer much from the disease and its conventional treatments’ side effects. In the past, clinical data showed that some complementary and alternative medicine (CAM) possessed anticancer abilities, but some clinicians and scientists have queried about the scientific validity of CAM due to the lack of scientific evidence. There is great demand in narrowing the knowledge gap to explore the scientific and evidence-based knowledge of CAM in the anticancer field. With this aim, an unparalleled undertaking of a book series is launched to structurally consolidate this area. Although acupuncture and moxibustion have been practiced for millennia, they have come under scientific investigation only recently. Acupuncture and moxibustion have been used clinically to treat cancer-related symptoms (e.g. cancer pain, anxiety, depression and insomnia), manage treatment-induced side effects (e.g. hot flashes, nausea and vomiting) and enhance immunity (e.g. blood cell count, lymphocyte and natural killer cell activity). This is the first book manages to provide comprehensive and updated scientific evidence of the effectiveness of acupuncture and moxibustion intervention for cancer care. Laboratory and animal studies have uncovered the mechanisms of acupuncture and moxibustion for cancer therapy. Promising results of most clinical trials show the efficacy and safety of acupuncture and moxibustion in cancer management. This book consists of fifteen chapters which address a range of important aspects that will impact on the application of acupuncture and moxibustion. An overview of the safety and side effects of acupuncture and moxibustion for cancer care are also covered. In addition, the integration of acupuncture with Western medicine in cancer treatment and recent clinical trials are included as well. Opinion leaders around the world have teamed up to pursue a thorough and up-to-date review of the area, this volume will contribute great to the cancer, clinical and academic community by providing evidence-based information on the efficacy of acupuncture and moxibustion as an anticancer therapy.
