"Approximately 80% of subfertile couples still have a chance to conceive naturally and the most important thing is to assess whether any form of medical assistance will increase their chance of pregnancy over continuing natural conception. In couples with male subfertility, IUI, IVF and ICSI are frequently used fertility treatments. Evidence-based guidelines on male subfertility are limited and the use of medically assisted reproduction is generally experience-based. In roughly 1% of subfertile couples the male partner is diagnosed with non-obstructive azoospermia. Men with NOA are able to father their own genetic child by using testicular sperm extraction (TESE) in combination with ICSI. Since both TESE and TESE-ICSI are invasive and costly procedures, patients should be well informed about their chances of successful TESE and their likelihood to conceive after TESE-ICSI before undergoing treatment. In this thesis, we performed several studies on male subfertility and MAR. We concluded that categorizing couples with male subfertility based on the TMSC allows for better classification in terms of chance of spontaneous ongoing pregnancy rate than categorization using the WHO classification system. We also concluded that there is insufficient evidence to determine whether there is any difference in safety and effectiveness between different treatment strategies for male subfertility. Furthermore, we developed a prediction model for obtaining sperm after testicular sperm extraction (TESE) in men suffering from NOA, as well as a prediction model for live birth in TESE-ICSI. Finally, we concluded that the current sperm DNA fragmentation tests have limited capacity to predict the chance of pregnancy in context of MAR."--Samenvatting auteur.

Antioxidants for Female Subfertility u2013 a Systematic Review BackgroundA couple may be considered to have fertility problems if they have been trying to conceive for over a year with no success. This may affect up to a quarter of all couples planning a child. It is estimated that for 40% to 50% of couples, subfertility may result from factors affecting women.Many subfertile women undergoing treatment also take dietary supplements in the hope of improving their fertility. This can be a very costly and stressful time for women and their partners. It is important that these couples be given high-quality evidence that will allow them to make informed decisions on whether taking a supplemental antioxidant when undergoing fertility treatment will improve their chances or cause any adverse effects. This is especially important, as most antioxidant supplements are uncontrolled by regulation. Antioxidants are thought to reduce the oxidative stress brought on by these conditions. Currently, limited evidence suggests that antioxidants improve fertility, and trials have explored this area with varied results. This review assesses the evidence for the effectiveness of different antioxidants in female subfertility. MethodsThe meta-analysis was performed as per the Cochrane manual. Databases were searched and studies were selected based on the following inclusion criteria: randomised controlled trials (RCTs) that compared any type, dose or combination of oral antioxidant supplement with placebo, no treatment or treatment with another antioxidant, among women attending a reproductive clinic. We excluded trials comparing antioxidants with fertility drugs alone and trials that only included fertile women attending a fertility clinic because of male partner infertility. The primary review outcome was live birth; secondary outcomes included clinical pregnancy rates and adverse events. ResultsWe included 50 trials involving6510 women. Investigators compared oral antioxidants, including combinations of antioxidants,N-acetyl-cysteine, melatonin, L-arginine, myo-inositol,D-chiro-inositol, carnitine, selenium, vitamin E, vitamin B complex, vitamin C, vitamin D+calcium, CoQ10, pentoxifylline and omega-3-polyunsaturated fatty acids versus placebo, no treatment/standard treatment or another antioxidant.Low-quality evidence suggests thatantioxidants may be associated with an increased live birth ratecompared with placebo or no treatment/standard treatment (OR 2.13, 95% CI 1.45-3.12, P > 0.001, 8 RCTs, 651 women). The overall objective of this research was to define the cellular basis underlying heritable subfertility in roosters homozygous for the rose comb allele (R/R). Fertilization in the hen is preceded by the ascension of motile sperm through the vagina and sperm sequestration within sperm storage tubules (SST). The objective of the first set of experiments was to determine if reduced sperm sequestration could account for subfertility. Sperm sequestration differed between genotypes following intravaginal insemination (p0.0001). However, sperm sequestration did not differ between genotypes when sperm were incubated with SST in vitro (p0.05). Therefore, subfertility was attributed to reduced sperm transport within the vagina. To test this hypothesis, an assay was developed to evaluate fowl sperm motility in vitro. Based upon this assay, ejaculates from subfertile males contained smaller subpopulations of highly motile sperm than the ejaculates from controls (p0.001). The objective of the next set of experiments was to characterize the motility of individual sperm and to identify a mechanism that could account for the genotypic difference in sperm cell motility. Computer-assisted sperm motion analysis evaluation revealed that ejaculates from controls contained 91% motile sperm whereas ejaculates from subfertile males contained 62% motile sperm (p0.001).
