"The first goal in this edition of the book remains as in the first edition - to cover the broad issues involved in the care of the spinal cord injured patient. The second goal is to provide an evaluation of spinal cord injury by experts who are deeply involved with various aspects of spinal cord injury management. Included in this new and revised edition are chapters devoted to three significant areas of development, particularly upper limb reconstruction, the use of electrical stimulation, and neuronal preservation after ischemic injury. New chapters review the state of exercise, standing, and walking systems using electrical stimulation, and the important and emerging topic of neuronal preservation after ischemic injury. The scope of this book includes: diagnostic methods evaluation methods spinal cord injury pathophysiology medical/surgical management rehabilitation and issues of specialized care This book is a compendium of otherwise difficult to assemble knowledge replete with time tested methods as well as with contemporary developments in the form of new ideas, techniques, and concepts."

Book Description :

"Spinal cord injury has devastating consequences for the person involved. An initial period of hospitalization is followed by a period of readjustment and rehabilitation in which the person with SCI must learn a new way of life. These changes have a cost that is borne by the person involved, their families and friends, and society at large. SCI costs the nation more than $9.73 billion per year. Each newly injured person with SCI incurs total first year costs that average $2.367 billion. Annual SCI-related medical care costs average $1.624 billion per year. Medications and supplies cost $449 million annually for those beyond their first year post-injury. The average cost of personal assistance is estimated to be $2.068 billion for those who are beyond the first year post-injury. These are the obvious direct costs that must be borne by persons with SCI or by someone on their behalf. When viewed from a societal point of view, SCI also imposes huge indirect costs. Such costs do not arise from anyone's purchase of goods or services but from the losses that come about as persons with SCI are forced to stop work or to reduce their productive activities. Taking these costs into account the authors estimate that: Losses of productivity, the indirect costs of SCI, are approximately $2.591 billion nationwide. Over the course of the post-injury lifetime, a person with SCI can expect to expend anywhere from $292,800 to $880,700 for injury-related costs, and to lose anywhere from $296,800 to $440,100 in lifetime earnings because of the injury. Data are needed on the details of these costs to provide guidelines for the allocation of funds to improve the quality of care post-injury and to ease the transition to work. The development of new technologies and improvement in the scope and timing of interventions requires both technical expertise and a realistic measure of their economic feasibility. The attempt to identify and quantify the costs of SCI is a step in the direction of supplying such information. This volume expands and updates information previously presented in 'The Economic Consequences of Traumatic Spinal Cord Injury,' reporting results from a new survey of the costs of spinal cord injury (SCI). It explores in detail the elements that make up both the medical and the nonmedical direct costs of SCI, focusing specifically on the costs of home modifications, vehicle adaptations, and wheelchairs. It also focuses on the determinants of employment for persons with this traumatic condition. The population sample was obtained by using lists supplied by hospitals that are members of the Spinal Cord Model Systems, together with names from the membership list of the Paralyzed Veterans of America. This volume is important reading for everyone who is involved in the acute management and long-term care of persons with SCI."

Book Description :

Not long after Indiana University had won the NCAA championship in 1981, a young man of twenty was hurriedly riding his bicycle in order to make it on time for a tennis tournament. He had plans for returning to the game after having been sidetracked with the "college life." Although he expected to attend graduate school, he was hoping to play professional tennis one day. He never made it to that tournament. A head-on collision with an automobile had crushed his dreams and also his neck, resulting in a cervical spinal cord injury. As he lay in the intensive care unit unable to move, he listened to music on his Walkman to distract him from his terrible predicament. His favorite tape, "The Lamb" by Genesis, seemed to help keep his attitude positive and hopeful. The following months are torturous and frustrating and he prays for a miracle; near-death experiences that seem too mysterious to comprehend show him that there is life beyond human existence. Then, after finally making it to the rehabilitation unit, he meets other young men in similar situations and they all struggle together to increase their functional abilities. In this rare and candid memoir, Stephen Thompson shares his many tribulations as he experiences new beginnings, both physical and spiritual, and strives for the ultimate goal of any spinal cord injury victim: to walk again.

Book Description :

In keeping with the aims of other books in this Series the Editors have concentrated on the practical aspects of management -in this case of the urinary tract in patients with spinal cord injury. It is well accepted that the management of such patients is best done by those with special experience in this field. Nevertheless, it is essential that urologists should be well informed on matters relating to the neuropathic urinary tract since not all patients will be managed in special centres and, whether their problems are acute or chronic, the wrong clinical decision can lead them into a lifetime of problems. The plan of this book is directed towards examining particular problems and providing definitive answers. Even in this enlightened age of medical progress there are many clinical situations where there is often a choice of treatment. Readers of this Series will be aware that, despite an apparent abundance of information on a topic, it is sometimes not possible to reach a firm conclusion on a management problem. Keith Parsons and John Fitzpatrick are to be congratulated on ensuring that for this subject there are good clear guidelines. The data in the book are up to date and the excellent approach to the subject makes it a valuable addition to Clinical Practice in Urology.
