Abstract: Oral health is important not only due to the diseases emerging in the oral cavity but also due to the direct relation to systemic health. Thus, early and accurate characterization of the oral health status is of utmost importance. There are several salivary biomarkers as candidates for gingivitis and periodontitis, which are major oral health threats, affecting the gums. These need to be verified and validated for their potential use as differentiators of health, gingivitis and periodontitis status, before they are translated to chair-side for diagnostics and personalized monitoring. We aimed to measure 10 candidates using high sensitivity ELISAs in a well-controlled cohort of 127 individuals from three groups: periodontitis (60), gingivitis (31) and healthy (36). The statistical approaches included univariate statistical tests, receiver operating characteristic curves (ROC) with the corresponding Area Under the Curve (AUC) and Classification and Regression Tree (CART) analysis. The main outcomes were that the combination of multiple biomarker assays, rather than the use of single ones, can offer a predictive accuracy of > 90% for gingivitis versus health groups; and 100% for periodontitis versus health and periodontitis versus gingivitis groups. Furthermore, ratios of biomarkers MMP-8, MMP-9 and TIMP-1 were also proven to be powerful differentiating values compared to the single biomarkers

This thesis focuses on the use of human serum albumin as a biomarker by providing means to assess oral health and leakage of blood biomarkers into saliva that may improve future saliva diagnostic assays. The use of saliva has emerged in diagnostic research due to the distinctive advantages offered by this biological fluid. This non-invasive biological fluid removes the need for a trained health care provider, making the collection process convenient and amenable to point-of-care testing. Moreover, recent extensive studies of biomolecules measured in saliva report their use to monitor health, response to therapy, and prognose both oral and systemic disease progression. Saliva is a complex mixture that results from multiple salivary glands surrounding the oral cavity. In addition to the secretions from the salivary glands, it is also prone to expectorated secretions from the bronchia and the nose, serum leakage from oral wounds, incoming pathogens with food debris, and peeled epithelial cells from the oral surface. Because of this complexity, method of saliva collection dictates the type and concentration of biomarkers measured. Although there are various saliva collection methods that have been developed in the effort to minimize the contamination of saliva from external impurities, very little research has been done to assess the contamination from serum leakage. The work from this thesis explores new means to more accurately determine the contribution of serum to the measurement of biomarkers in saliva by using human serum albumin. Human serum albumin is the most abundant serum protein that is found at relatively much lower concentrations in saliva. Its close connection with the oral surface and the diffusion of serum components into saliva, high levels of human serum albumin in saliva has been known to be linked to oral health. Considerably high concentrations of human serum albumin in saliva are examined in this thesis to define whether human serum albumin can be used to develop a simple test to define the serum leakage rate and oral health.

The salivary proteome is recognized as a valuable source of potential oral and systemic disease biomarkers. Major efforts in salivary research have been dedicated to identify and characterize salivary proteins present in saliva using both classical biochemical methods and proteomics approaches in adults. Despite considerable research on the salivary proteome, little attention has been given to the changes in the salivary proteome occurring in children, specifically from 0-3 years of age. Through the use of anionic PAGE, SDS PAGE, HPLC and MS/MS, salivary protein profiles in children before, during and after dental eruption were compared with edentulous adult controls. We identified substantive qualitative and quantitative differences in the salivary proteome between children and adults, suggesting a greater emphasis is warranted in the study of the changes in the salivary proteome as a function of age and dental status.
