Book Description :

Gain expertise in primary care of older adults with a case-based approach to geriatric primary care and multimorbidity management. Written by two leading academic and clinical experts in geriatric primary care, Case Studies in Geriatric Primary Care and Multimorbidity Management, 1st Edition uses detailed Exemplar Case Studies and Practice Case Studies to teach you how to think like an expert geriatric clinician. Because most older adults have more than one condition when seeking care, both Exemplar and Practice Case Studies place a strong emphasis on "multimorbidity" management, (the management of patients with a host of complex, interacting conditions). To provide extensive practice in learning how to think like an expert, case studies reflect the reality that care does not necessarily begin or end in the primary care setting. Cases therefore move fluidly from primary care to acute care to inpatient rehabilitation to assisted living to long-term care. Building on foundational introductory chapters, cases also call on you to develop interprofessional collaboration skills and reflect the diversity of today's older adults, in terms of age (young-old to old-old), gender, culture, ethnicity, sexuality, socioeconomic status, and more! As you work through both basic-level and advanced Practice Case Studies, you can make extensive notes in the printed book and then go online to submit answers for grading and receive expert feedback for self-reflection. NEW! Introductory unit on the core principles of caring for older adults gives you a strong foundation in the principles of geriatric primary care and multimorbidity management. NEW! and UNIQUE! Exceptionally detailed, unfolding Exemplar Case Studies demonstrate how an expert advanced practitioner "thinks clinically" to provide care to older adults with multiple conditions. NEW and UNIQUE! Exceptionally detailed, unfolding Practice Case Studies emphasize patient diversity and multimorbidity management across healthcare settings to help you develop advanced clinical reasoning skills for geriatric primary care. NEW and UNIQUE! Strong emphasis on multimorbidity management focuses on caring for older adults with multiple chronic conditions. NEW! Emphasis on the continuum of care across settings reflects the reality that care does not necessarily begin or end in the primary care setting but can move from primary care to acute care to inpatient rehabilitation to assisted living to long-term care, and so forth. NEW! Online answer submission for grading and expert feedback for self-reflection. NEW! Emphasis on patient diversity reflects the makeup of today's older adult, population in terms of age (young-old to old-old), gender, culture, ethnicity, sexuality, socioeconomic status, and more. Book Description :

As the Baby Boomers age, concerns over healthcare systems' abilities to accommodate geriatric patients grow increasingly challenging. This is especially true with the population deemed to be “the oldest of the old,” specifically those over the age of 85. Unlike any other time in history, this demographic is the fastest growing segment of most developed countries. In the United States the oldest old is projected to double from 4.3 million to 9.6 million by 2030. The increased life expectancy of the population since the early 1900s has been built on the improvement of living conditions, diet, public health and advancement in medical care. With this we have seen a steady decline in the age-specific prevalence of vascular and heart diseases, stroke and even dementia. Older persons are healthier today than their counterparts decades ago. More importantly than in any other age group, the care of the oldest old must be individualized; management decisions should be made taking into consideration the older persons’ expressed wishes, quality of life, function and mental capacity. The inevitable consequence is that there will be an increase in the prevalence of older persons with chronic diseases, multiple co-existing pathologies and neuro-degenerative diseases. The oldest of the aging population are often excluded from drug trials and their treatments are largely based on findings extrapolated from that of the younger old. Furthermore, among the oldest old, physiologically they are more diverse than other segments of the population. Their demographic characteristics are unparalleled and different compared to that of the younger old. Several studies have drawn attention to the differing attitudes among health professionals towards elderly people and many show prejudice because they are old. As a result, the use of age as a criteria in determining the appropriateness of treatment is of very limited validity, yet there are limited resources that guide physicians through these challenges. This book creates a greater awareness of these challenges and offers practical guidelines for working within the infrastructures vital to this demographic. This book is designed for geriatricians, primary care physicians, junior medical officers, specialty geriatrics nurses, and gerontologists. It is divided into 3 sections: General Considerations, Chronic diseases and Geriatric Syndromes. Each chapter provides a summary of important and essential information under the heading of Key Points. Case studies are included in some of the chapters to highlight the principles of management.

Book Description :

Integrative geriatrics is a new field of medicine that advocates for a whole-person, patient-centered, primarily non-pharmacological approach to medical care of the elderly. Most current geriatric practices overprescribe medications and procedures and underutilize non-pharmacological, low-cost, high-touch methods. Patients, however, often show reluctance towards these standard practices, as the interventions they rely upon are often invasive. The practice of integrative geriatrics is rooted in lifestyle interventions, such as nutrition, movement therapies, and mind-body and spirituality approaches, that allow patients to have different path to their healthcare-one that utilizes pharmaceuticals and invasive procedures only when safer integrative approaches are not available or not effective. This first definitive textbook of this new field presents detailed, evidence-based information for all healthcare providers and advocates who work with geriatric population. It is intended not only for providers in outpatient settings, but also to those who work in nursing homes, assisted and independent living facilities, and senior community centers. In addition, Integrative Geriatric Medicine will provide valuable information for leaders and politicians who are involved with implementing policies and procedures for care of elderly patients looking for safer, less costly, and more patient-centered approaches.

Book Description :

This book provides family doctors with a wealth of evidence-based indications and tips regarding geriatric medicine and approaches for the management of older patients, to be applied in daily practice. After discussing old and new features of healthy ageing and the approaches required in Family Medicine Consultation, the text introduces key elements of geriatric medicine such as frailty, sarcopenia, and the comprehensive geriatric assessment (CGA), before describing a range of characteristics unique to older patients in different contexts, with a dedicated section on Palliative Care. The role of polypharmacy and the importance of quaternary prevention and deprescribing are also addressed. Finally, the book emphasizes both the importance of a humanistic approach in caring and the approach of research and meta-research in geriatrics. Though many texts explore the role of primary care professionals in geriatric medicine, the role of family doctors in older people care has not yet been clearly addressed, despite the growing burden of ageing, which has been dubbed the “silver tsunami.” Family physicians care for individuals in the context of their family, community, and culture, respecting the autonomy of their patients. In negotiating management plans with their patients, family doctors integrate physical, psychological, social, cultural and existential factors, utilizing the knowledge and trust engendered by repeated visits. They do so by promoting health, preventing disease, providing cures, care, or palliation and promoting patient empowerment and self-management. This will likely become all the more important, since we are witnessing a global demographic shift and family doctors will be responsible for and involved in caring for a growing population of older patients. This book is intended for family medicine trainees and professionals, but can also be a useful tool for geriatricians, helping them to better understand some features of primary care and to more fruitfully interact with family doctors.

Book Description :

This textbook presents hands-on training material for medical students. The style reflects the need for practice-based teaching with a modern edge in daily clinical routine; accordingly, it also employs online material and pocket cards. Each chapter begins with specific learning objectives, which are cross-referenced with the European curriculum for undergraduate medical education released by the European Union of Medical Specialists (UEMS) together with the European Union Geriatric Medicine Society (EUGMS), as well as the minimum geriatric competences for medical students established by the American Geriatrics Society (AGS). World-renowned European experts in practicing and teaching the interdisciplinary field of Geriatrics contributed to this work, with the aim of offering the new generation of health professionals a global perspective on one of the greatest public health challenges of our time: the management of the steadily increasing number of older, multimorbid, and vulnerable persons. The major strength of this book – published under the auspices of the EUGMS – is its pragmatic, goal-oriented approach, which makes it suitable for bedside learning and patient-centered medicine; further, all of the chapters are firmly based on the pillars of the ageing process in all of its biological aspects, helping readers understand the pathophysiology of and rationale behind interventions for the main geriatric syndromes and disorders.

In an ageing population, those who specialise in the treatment of older people are more essential than ever. The specialty is also becoming more evidence-based as research into care of the elderly grows. This second edition of Essential Geriatrics has been comprehensively updated and expanded, with entirely new sections on the ageing process and a selection of important topics. In addition there are 100 questions throughout the text in the modern knowledge-based 'best of five' format, with answers. This text specifically focuses on the key aspects of elderly care, spanning both general medicine and the particular problems encountered in geriatrics as a route into the specialty: the subjects chosen typically receive scant coverage in general medical texts. Practical guidance for managing the conditions covered is included at the end of each section, with reasoned approaches to areas where evidence is more scarce. Over the past two decades there has been a marked change in global age demographics, with the number of over-60s increasing by 82% and the number of centenarians by 715%. This new-found longevity is testament to the success of recent advances in medicine, but poses significant challenges to multiple areas of health care concerning older patients. Fully revised to reflect the current state of geriatric medicine, it examines the medical and scientific basis of clinical issues, as well as the ethical, legal, and socio-economic concerns for healthcare policy and systems. Over 170 chapters are broken up into 16 key sections, covering topics ranging from policy and key concepts through to infection, cancer, palliative medicine, and healthy ageing. New material includes focus on the evolving concepts of malnutrition, sarcopenia, frailty, and related geriatric syndromes and integration of geriatric principles from public health, primary and specialized care, and transitional stages from home to emergency, medicine and surgery, rehabilitation, and long term care.
