The electro-optic technique of measuring the cardiovascular pulse wave known as photoplethysmography (PPG) is clinically utilised for noninvasive characterisation of physiological components by dynamic monitoring of tissue optical absorption. There has been a resurgence of interest in this technique in recent years, driven by the demand for a low cost, compact, simple and portable technology for primary care and community-based clinical settings, and the advancement of computer-based pulse wave analysis techniques. PPG signal provides a means of determining cardiovascular properties during the cardiac cycle and changes with ageing and disease. This thesis focuses on the photoplethysmographic signal for cardiovascular assessment. The contour of the PPG pulse wave is influenced by vascular ageing. Contour analysis of the PPG pulse wave provides a rapid means of assessing vascular tone and arterial stiffness. In this thesis, the parameters extracted from the PPG pulse wave are examined in young adults. The results indicate that the contour parameters of the PPG pulse wave could provide a simple and noninvasive means to study the characteristic change relating to arterial stiffness. The pulsatile component of the PPG signal is due to the pumping action of the heart, and thus could reveal the circulation changes of a specific vascular bed. Heart rate variability (HRV) represents one of the most promising quantitative markers of cardiovascular control. Calculation of HRV from the peripheral pulse wave using PPG, called pulse rate variability (PRV), is investigated. The current work has confirmed that the PPG signal could provide basic information about heart rate (HR)and its variability, and highly suggests a good alternative to understanding dynamics pertaining to the autonomic nervous system (ANS) without the use of an electrocardiogram (ECG) device. Hence, PPG measurement has the potential to be readily accepted in ambulatory cardiac monitoring due to its simplicity and comfort. Noncontact PPG (NPPG) is introduced to overcome the current limitations of contact PPG. As a contactless device, NPPG is especially attractive for physiological monitoring in ambulatory units, NICUs, or trauma centres, where attaching electrodes is either inconvenient or unfeasible. In this research, a prototype for noncontact reflection PPG (NRPPG) with a vertical cavity surface emitting laser (VCSEL) as a light source and a high-speed PiN photodiode as a photodetector is developed. The results from physiological experiments suggest that NRPPG is reliable to extract clinically useful information about cardiac condition and function. In summary, recent evidence demonstrates that PPG as a simple noninvasive measurement offers a fruitful avenue for noninvasive cardiovascular monitoring.

Arterial Blood Pressure is one of the primary indicators used to monitor health. It is often useful to continuously observe fluctuations in the systolic and diastolic blood pressure in post-operative patients. Conventional devices like a sphygmomanometer are non-invasive methods of reading blood pressure, but are not continuous. Another commonly used technique, which is invasive, is an intra-arterial blood pressure sensing mechanism. A non-invasive method to continuously track variations in blood pressure is very useful as it has the advantages of not being intrusive and hence eliminating the need to surgically implant a device to sense intra-arterial pressure and the risk of infection. In this project, different methods of processing the photoplethysmogram signal are analyzed with the aim of identifying the components of the signal that vary with blood pressure, and can distinctly report the systolic and diastolic values. The data collected from 20 subjects is statistically analyzed. Pressure was also calculated from measured photoplethysmogram, using principles from photonics and biomechanics. This method was found to be the closest approximation to real-time pressure. Analysis of different parameters of the photoplethysmogram indicates that even though it depends upon pressure to a certain extent, there is no simple relationship that is valid or consistent over a reasonable period of time. However, with some improvements to the sensitivity of the device and by using the calculated pressure from the photoplethysmogram, the real-time blood pressure can be measured.

Purpose: The photoplethysmogram (PPG) recorded using finger pulse oximeter estimates the oxygen saturation. During out-of-hospital cardiac arrest (OHCA) the PPG waveform shows amplitude changes with chest compressions (CC. The aim of this study was to analyze the feasibility of using the finger PPG to provide feedback on CC rate during OHCA.Materials and methods: Records of 46 patients were included, comprising the PPG and compression depth signals extracted from Zoll E-Series defibrillator files. They were acquired by the DFW Center for Resuscitation Research (UTSW, Dallas) and the Emergency Medical Services agencies from the Resuscitation Outcomes Consortiumu2019s Portland site (OHSU). An automatic algorithm analyzed the PPG every 5s and detected useful PPG, presence of CCs and CC-rate. The compression depth signal (acquired by the CPR-pad-z) was used as the reference signal to simultaneously compute the rate and check for presence of CCs. The percentage of useful PPG, and sensitivity (SE)/positive predictive value (PPV) for detecting CCs were computed. The CC-rate error was assessed as mean(standard deviation) and 95% level of agreement (LOA, Bland-Altman). A feedback system on the CC-rate was implemented for the intervals where PPG was available, and evaluated in terms of SE, PPV and specificity (SP) to trigger alarms for CC-rates out of the range 90-120 CC/min.Results: A total of 420 min (9.1u00b17.6 min per episode) were processed. A total of 4971 segments were analyzed, 4319 including CCs with rate 116 (15) CC/min. The PPG signal was not available 17% of the time. The SE/PPV to detect CCs were 86.2/95.4%, and the CC-rate error 2.4 (6.3) CC/min (see figure). The SE/SP/PPV for the feedback system to trigger alarms were 83.6/92.4/85.3% respectively. Conclusions: When available, the finger PPG signal reliably gives feedback on CC rate.
