Asthma, chronic pulmonary disease, is one of the major health issues in the United States. Given its chronic nature, the demand for continuous monitoring of patient's adherence to the medication care plan, assessment of their environment triggers, and management of asthma control level can be challenging in traditional clinical settings and taxing on clinical professionals. A shift from a reactive to a proactive asthma care can improve health outcomes and reduce expenses. On the technology spectrum, smart conversational systems and Internet-of-Things (IoTs) are rapidly gaining popularity in the healthcare industry. By leveraging such technological prevalence, it is feasible to design a system that is capable of monitoring asthmatic patients for a prolonged period and empowering them to manage their health better.In this thesis, we describe kBot, a knowledge-driven personalized chatbot system designed to continuously track medication adherence of pediatric asthmatic patients (age 8 to 15) and monitor relevant health and environmental data. The outcome is to help asthma patients self manage their asthma progression by generating trigger alerts and educate them with various self-management strategies. kBOT takes the form of an Android application with a frontend chat interface capable of conversing both text and voice-based conversations and a backend cloud-based server application that handles data collection, processing, and dialogue management. The domain knowledge component is pieced together from the Asthma and Allergy Foundation of America, Mayoclinic, and Verywell Health as well as our clinical collaborator. Whereas, the personalization aspect is derived from the patient's history of asthma collected from the questionnaires and day-to-day conversations. The system has been evaluated by eight asthma clinicians and eight computer science researchers for chatbot quality, technology acceptance, and system usability. kBOT achieved an overall technology acceptance score of greater than 8 on an 11-point Likert scale and a mean System Usability Score (SUS) greater than 80 from both evaluation groups.

Macrophages are core components of the innate immune system. Once activated, they may have either pro- or anti-inflammatory effects that include pathogen killing, safe disposal of apoptotic cells or tissue renewal. The activation state of macrophages is conceptualized by the so-called M1/M2 model of polarization. M2 macrophages are not simply antagonists of M1 macrophages; rather, they represent a network of tissue resident macrophages with roles in tissue development and organ homeostasis. M2 macrophages govern functions at the interfaces of immunity, tissue development and turnover, metabolism, and endocrine signaling. Dysfunction in M2 macrophages can ruin the healthy interplay between the immune system and metabolic processes, and lead to diseases such as insulin resistance, metabolic syndrome, and type 1 and 2 diabetes mellitus. Globally, severe asthma is defined by the WHO as either (A) untreated severe asthma; (B) difficult-to-treat severe asthma; and (C) treatment-resistant severe asthma. Untreated severe asthma is a political problem: the children do not have access to the basic tools for asthma management, and when this is corrected, asthma outcomes are transformed. The problem in difficult-to-treat severe asthma is not the airway disease, but co-morbidities and behavioral factors. This is the group in which there are most asthma deaths, underscoring that severe asthma cannot be solely defined by levels of prescribed therapy. Treatment-resistant severe asthma is rare and challenging, and the problem is the airway pathology. These children require new and innovative therapies.

Asthma is a common complex and heterogeneous respiratory disease with an increasing prevalence in developed countries. Asthma is a disease consisting of different phenotypes that are driven by different mechanistic pathways (endotypes). The recognition of these phenotypes and endotypes is central to asthma management entailing prognostic and therapeutic implications. It is acknowledged that despite optimal treatment, many patients are poorly controlled, highlighting the need for phenotype-guided treatments. In this context, the emergence of novel therapies (monoclonal antibody therapy, bronchial thermoplasty) is paving the way for personalized asthma therapy. A better understanding of disease pathogenesis may enable the identification of biomarkers, mediators, novel therapeutic targets, and treatable traits. Further molecular phenotyping or endotyping of asthma will be necessary to tailor new therapeutic strategies. The present Special Issue on Asthma aims to provide the current knowledge on phenotypes and endotypes in appreciating and managing the heterogeneous condition that is asthma.
