Peripartum cardiomyopathy (PPCM) is a rare but devastating disease that hits women at a time when heart disease should not be a concern. PPCM is a type of idiopathic, non-ischemic, dilated cardiomyopathy presenting with symptoms of heart failure primarily due to left ventricular (LV) systolic dysfunction that is manifested in the last trimester of pregnancy and up to 5 to 6 months after delivery. It is clear from the literature that women from black or Haitian descent are the most prone to having the condition. In Lebanon, no studies are conducted on the disorder in general. The clinical presentation can be either sudden requiring urgent admission to the intensive care unit, progressive or latent. The etiopathogeneis is still unknown but thought to be multifactorial. Signs and symptoms are similar to those in acute heart failure. The diagnosis is a diagnosis of exclusion, after ruling out cardiac-pregnancy related conditions. Echocardiography is the gold standard tool for diagnosis and cardiac magnetic resonance can be added as a complementary in critical unresponsive cases. Treatment is similar to that of conventional heart failure with consideration of contraindicated drugs in pregnancy and lactation. Novel drugs and biomarkers are promising but still in the early stages and more studies are still required to finalize and generalize their bedside uses. Since early diagnosis is the key to early recovery with less complication and preservation of ejection fraction, I shed the light on screening and risk stratification. Therefore, I created a detailed clinical pathway for early diagnosis, risk factors, differential diagnosis, complications, and management. This will help health care providers to detect the disease early enough to avoid devastating outcomes.

Problem Statement: Peripartum Cardiomyopathy (PPCM) is an unusual and uncommon form of non-ischemic dilated cardiomyopathy that presents in the last month of pregnancy or within 5 months after delivery in the absence of other causes. The incidence of PPCM is 1 in 3,000 normal pregnancies with mortality rate as high as 50% to 75%. The etiology of PPCM could be many potential causes, including viral myocarditis, abnormal immune response in pregnancy and abnormal response to the increased hemodynamic burden of pregnancy. The occurrence of PPCM in woman with pre-eclampsia is a very rare case. This case series present two women with pre-eclampsia underwent Cesarean Section and aufb00ected by PPCM. The limitation of data and the rarity of similar case cause diufb03culties in the management.Method: Case SeriesResult:Peripartum Cardiomyopathy related to more severe effect for women with pre-eclampsia who underwent Cesarian Section and might be influenced by excess secretion of anti-angionic factor from the placenta. In this case series, one woman survives, but another woman died in ICU due to organ failure.Conclusion:Peripartum Cardiomyopathy occurred after Cesarean Section in woman with pre-eclampsia can cause very severe eufb00ect and even death.

Abstract : Abstract: Peripartum cardiomyopathy (PPCM), often classified as a form of dilated cardiomyopathy (DCM), is the myocardial dysfunction that occurs in late pregnancy and through the first few postpartum months. The aim of this study is to investigate the differences in the clinical outcomes of PPCM and DCM. Electronic medical records from 1997 to 2011 were retrieved from the Taiwan National Health Insurance Research Database. Patients with PPCM were compared with age- and clinical characteristics-matched patients with DCM. Primary outcomes were 1- and 3-year heart failure (HF) readmission, cardiac death, all-cause mortality, and major adverse cardiovascular events. Secondary outcomes were myocardial infarction, new onset of dialysis, heart transplant, and cerebrovascular accident. Follow-up period was divided into "within the first year" and "after the first year." A total of 527, 979 patients (253, 166 females) were hospitalized with a principal diagnosis of HF during 1997 to 2011 period. After excluding patients aged 18 and 50 years, patients with other forms of HF, and those with a history of cerebrovascular accidents or coronary artery disease, 797 patients with PPCM and 1267 patients with DCM were evaluated. Propensity score matching yielded 391 patients in each group. Patients with DCM had a significantly worse prognosis compared to those with PPCM for all primary and secondary outcomes at the 1- and 3-year follow-ups. After 1 year, the HF readmission rate did not significantly differ between the 2 diseases, suggesting that HF medications should be aggressively instituted in patients with PPCM. This is the first study to directly compare the clinical outcomes between age-matched patients with PPCM and DCM. Patients with PPCM had a significantly better prognosis across all cardiovascular endpoints compared to patients with DCM.
