Book Description :

Abstract: This qualitative research study describes the experiences of child and adolescent liver transplant recipients and their parents during the patients' transition to adulthood. Data were collected from pediatric liver transplant recipients and their parents during individual interviews, and these were later analyzed using conventional content analysis. Seven main themes emerged: coping, self‐management, body image, social relationships, academic life, work life, and live donors. Study results revealed that the patients who received liver transplants during their childhood and adolescence used both effective and ineffective strategies to cope with the difficulties they faced during the transition period into adulthood. The parents experienced many problems: on learning of their child's need for a transplant, parents were advised that they should consider becoming a live donor. This very difficult decision was a source of great stress and required serious consideration. After transplantation the parents wanted their children to have autonomy but could not encourage them because of concerns for their health.

Book Description :

This book is the first to provide balanced examination of both pediatric liver disease and liver transplantation – two topics that are inherently related, given that most chronic liver disorders eventually require organ replacement. The different forms of liver disease encountered in the pediatric age group are first discussed in a series of disease-specific chapters that have a reader-friendly, uniform structure covering pathophysiology, diagnostic and treatment algorithms, clinical cases, and transition to adult care. Key topics in the field of liver transplantation are then addressed. Examples include indications and contraindications, surgical techniques and complications, immunosuppression, in pediatric liver transplantation, acute and chronic rejection and allograft dysfunction, and CMV and EBV infection in transplant recipients, long-term graft injury and tolerance. A section on pediatric hepatology across the world includes chapters presenting the features and management of pediatric liver disease in South-America, Africa and Asia. A closing section considers what the future holds for pediatric liver disease and its management, including novel genetic testing, cell therapy and gene therapy. Pediatric Hepatology and Liver Transplantation will be of value for a range of practitioners, from residents making their first approach to pediatric liver disease through to specialists working in transplantation centers.

Book Description :

Background and Aims: In recent years, liver transplantation (LT) has become a well-accepted therapeutic modality for children with end stage liver disease, with transplantation surgery being performed at a younger age. Primary infection with human herpes virus 6 (HHV-6) occurs in most children within the first two years of life, therefore, data on primary HHV-6 infection in pediatric liver transplant recipients is scarce. Our objective was to describe the course of primary HHV-6 infection after pediatric LT.Methods: Medical files, between the years 2015-2016, of post-LT pediatric patients with suspected primary HHV-6 infection were reviewed. Clinical and laboratory data for enrolled cases were evaluated. Results: Four cases of primary HHV-6 (type B) infection were identified amongst the 26 children who had undergone LT at our center during the study period. All patients were

Book Description :

A number of areas within liver transplant continue to evolve as the field matures. The advent of increasingly effective therapy for HCV will dramatically reduce the burden of recurrent disease within the next few years. HCC has become a well established indication for transplant with an increasing emphasis on adjuvant therapies to reduce the risk of tumor recurrence and potentially expand the acceptable limits of tumor burden amenable to liver transplant. Renal dysfunction is a frequent complication of decompensated liver disease and an important predictor of diminished post-transplant survival. Newer approached to therapy including terlipressin will reduce the impact of renal dysfunction in liver transplant candidates and potentially reduce the need for combined liver/kidney transplant. Increasing attention is being paid to role of comorbidities such as diabetes and hypertension on longterm recipient outcomes. Retransplantation remains a challenge with a variety of models available ot redict outcomes. Cardiac disease pretransplant is an area of active investigation with newer entities such as cirrhotic cardiomyopathy a growing concern. These are some of many evolving issues in liver transplant which will be addressed to update our readers and which in turn will enhance the care of patients with liver disease.
