Nocturia is the most common cause of interrupted sleep affecting roughly 50 million Americans and costs the United States an estimated $62.5 billion annually. It is associated with decreased quality of life, increased depressive symptoms, increased fall risk, decreased sleep duration and quality, increased risk for cardiovascular disease, and a 22% greater risk of all-cause mortality. PURPOSE: To consolidate mechanisms of nocturia stemming from conditions associated with aging, including cardiac failure, diabetes, circadian dysregulation of arginine vasopressin (AVP), and hypertension, and to provide evidence of the potential benefits of exercise as contrary to the age-associated conditions and mechanisms of nocturia. RESULTS: 474 articles were considered, 410 of which were excluded (n=64). 12 publications for cardiac failure, 17 for diabetes, 13 for AVP, and 22 for hypertension were included. The mechanisms of nocturia stemming from cardiac failure, diabetes, circadian dysregulation of AVP, and hypertension were increases in natriuretic peptides, disruption in circadian sodium and AVP secretion, reduction in AVP sensitivity, osmotic diuresis, storage symptoms, peripheral neuropathy, polydipsia, increased GFR, sympathetic nervous activation, edema, increased sympathetic response, and OSA. Sex differences exist in number of studies presented and exercise data was skewed toward men. CONCLUSIONS: Human aging produces physiological changes that influence the prevalence of cardiac failure, DM, circadian dysregulation of AVP, and hypertension. These conditions can produce numerous interwoven downstream effects, which can clarify why some older adults experience nocturia. Opposingly, we know that physical activity is a proven preventative and treatment behavior for many aging related diseases, including hypertension, cardiac failure, diabetes, and circadian dysregulation. However, despite these identified relationships, the connection between physical activity and the prevention and treatment of nocturia has only been made in a limited capacity. Outlined in this review are downstream pathways of age-associated conditions through which physical activity may be able to attenuate nighttime urine production have been made more clear. We find it even more exciting that specific types of exercise such as head-out of water “aqua-robics” or HIRT may have particular utility within an aged population experiencing nocturia. It is our hope that this information will be used in future interventions that specifically address their effectiveness in reducing the number of nocturnal polyuria and nocturia in older adults.
