Invertebrate animals represent a diversity of solutions to life's challenges. Success in a wide range of environments has been achieved by an almost bewildering range of invertebrate body forms. These body forms are reflected in the wonderful diversity of their nervous systems. Despite this apparent diversity, studies of the development of invertebrates and vertebrates are yielding common themes at the molecular level. Likewise, the phenome non of neural regeneration is based upon properties intrinsic to neurons and responses to a remarkably conserved chemical lan guage. This monograph focuses on the diversity and commonal ity of responses to neural injury. The rough and tumble of life may frequently damage some part of the body, particularly the appendages or sensory sys tems. The nervous system is usually involved in repair of other body systems and often may itself require repair. Some animals are particularly successful in regenerating the nervous system or body parts. We particularly marvel at these feats of regeneration because we human beings are not particularly successful, despite our relatively long life and the advantages that would seem to accrue from such repair. It is no wonder that we would hope to learn the secrets of the more successful animals and strive to emulate them! Mechanisms of neural regeneration are often more acces sible in invertebrates than in vertebrates because questions of specificity are more easily addressed using the identifiable neu rons of the relatively simpler nervous systems of some inverte brates.

Peripheral nerve injuries are a high-incidence clinical problem that greatly affects patients' quality of life. Despite continuous refinement of microsurgery techniques, peripheral nerve repair still stands as one of the most challenging tasks in neurosurgery, as functional neuromuscular recovery is rarely satisfactory in these patients. Therefore, the improvement of surgical techniques and the clinical application of innovative therapies have been intensively studied worldwide. Direct nerve repair with epineural end-to-end sutures is still the gold standard treatment for severe neurotmesis injuries but only in cases where well-vascularized tension-free coaptation can be achieved. When peripheral nerve injury originates a significant gap between the nerve stumps, nerve grafts are required, with several associated disadvantages. Therefore, the development of scaffolds by tissue engineering can provide efficient treatment alternatives to stimulate optimum clinical outcome. Nerve conduit tailoring involves reaching ideal wall pores, using electrospinning techniques in their fabrication, surface coating with extracellular matrix materials, and adding of growth factors or cell-based therapies, among other possibilities. Also, intraluminal cues are employed such as the filling with hydrogels, inner surface modification, topographical design, and the introduction of neurotrophic factors, antibiotics, anti-inflammatories and other pharmacological agents. A comprehensive state of the art of surgical techniques, tissue-engineered nerve graft scaffolds, and their application in nerve regeneration, the advances in peripheral nerve repair and future perspectives will be discussed, including surgeons' and researchers' own large experience in this field of knowledge.

Regenerative therapies for peripheral nerve injury have achieved significant success in neural regeneration by combining growth-permissive materials with anisotropic features to guide regenerating neurites across the injury gap. Despite their success in the peripheral nervous system, these strategies tend to neglect the neuron-level precision required for fully functional regeneration. Fortunately, studies of the developing peripheral nervous system have revealed a variety of innate molecular-level tools to promote outgrowth, guide axons toward targets of re-innervation, and stimulate re-myelination by glial cells. We have developed a class of customizable, protein-engineered microenvironments to study three aspects of the neural microenvironment involved in regeneration: cell-adhesive ligands in the Extracellular Matrix (ECM), soluble neurotrophic factors, and cell-cell contact. In particular, we demonstrated the ability of RGD ligand density in the ECM to enhance neurite outgrowth from Dorsal Root Ganglia. Furthermore, the strategic inclusion of Nerve Growth Factor in these microenvironments enhanced L1CAM-mediated interaction between Schwann cells and neurons, resulting in cooperative outgrowth between the two cell types. Microenvironments promoting both cell-cell and cell-matrix interactions resulted in better neural outgrowth than those that prioritized either one or the other. Finally, we incorporated a chemoattractive gradient of Nerve Growth Factor to induce directed outgrowth within 3-dimensional protein matrices. The density of cell-adhesive RGD ligands in the microenvironment impacted the ability of neurites to initiate outgrowth, progress through the matrix, and turn toward the chemoattractive source. These findings underscore the ability of orthogonal strategies to synergistically enhance neural regeneration, and the utility of protein-engineered microenvironments to apply those strategies.

Acidic Fibroblast Growth Factor (aFGF), which is identical to Retina Derived Growth Factor (RDGF), is a growth factor that stimulates neural differentiation and the formation of neurites. Work performed under this contract is determining if a gradient of aFGF can stimulate neurites growth in the direction of increasing aFGF concentrations. Preliminary experiments establishing conditions for a slow release of aFGF over a period of several days to create such a gradient have been established. Subsequent work will determine if PC12 cells or rat sensory cells will extend neurites in response to this gradient. We are also determining if aFGF can stimulate nerve regeneration in an in vivo animal model that measures nerve regeneration in a transected siatic nerve through a nerve guide tube. These experiments are providing evidence that purified aFGF can stimulate regeneration and that this regeneration is primarily due to an increase in neurite formation by sensory nerves. Neurogenesis.
