The Second World War gave rise to a previously inconceivable number of peripheral nerve injuries. Only later on did these injuries occasion renewed intensive research in this field of neurosurgery. Among the factors which have promoted this development is the operating microscope with more refined surgical techniques. Of course, surgery of peripheral nerve injuries in peacetime is not to be compared with that in wartime. Only rapid wound healing enables a secondary suture to be performed at a favorable time, after about four weeks. Besides in most cases the defect of substance is not as great as in war injuries, in which the so-called "critical resection length" (Seddon) was the cause of the greatest difficulties and consti tuted the crucial obstacle to good success. The experienced surgeons of the Second World War always knew that the specified dimensions were far too great for the critical resection length. They could only resort to attempting a suture even when there was a great gap be tween the ends of the nerves. Precise information was not available on direct damage to the axis cylinder or alteration due to vascular factors when stretching the various nerves, above all during the later mobilization of joint decompressions. In my opinion, the experimental investigations of the author can make a contribution here. The precisely executed and afterwards carefully analyzed experiments appear very likely to be applicable to human conditions as regards their percentages of total nerve length.

Wallerian degeneration has been extensively studied in models of peripheral and central nerve injuries and diseases. Degeneration of the severed distal fragment has been considered an essential preparatory stage to the process of axonal regeneration. In fact, an important reason why regeneration is stunted in the central nervous system (CNS) is attributed in part to the low efficiency of clearance ofinhibitory molecules that obstruct regrowth. Conversely, in the peripheral nervous system (PNS), injured nerve fragments rapidly degenerate and are efficiently removed. Infiltrating CCR2+ monocyte-derived macrophages are thought to be indispensable for nerve debris clearance after injury. It was therefore unexpected when our lab discovered that the Ccr2-/- mouse, which lacks the primary receptor forinfiltration of hematogenous macrophages, displayed comparable myelin and axonal clearance to wild-type (WT) mice throughout a time course after sciatic nerve transection injury. This compensatory clearance mechanism was not an effect of constitutive loss of CCR2 throughout development because mice treated with an anti-CCR2 antibody (MC-21) displayed comparable myelin clearance to both WTand global Ccr2-/- nerves. Further characterization of the molecular and cellular differences between WT and Ccr2-/- mice revealed important differences both at the level of the axotomized neuronal cell bodies and at the distal nerve. We found that Ccr2-/- nerves display significantly higher levels of mRNA for the neutrophil chemokines CXCL1 and CXCL2, and a concomitant increase in the number ofinfiltrating neutrophils compared to WT nerves after injury. Phagocytosis assays revealed that neutrophils in Ccr2-/- mice compensate for the loss of macrophages to facilitate efficient Wallerian degeneration. Furthermore, nearly complete depletion of neutrophils using an anti-Ly6G blocking antibody severely inhibited myelin clearance in both WT and Ccr2-/- nerves, illuminating a pivotal role for these cells in mediating Wallerian degeneration. Interestingly, preliminary in vivo experiments indicate that nerve regeneration following injury is uninhibited in Ccr2-/- mice, and that neutrophil depletion may retard regeneration in Ccr2-/- mice, but not WT mice. These data could suggest that neutrophils may be playing a redundant role in supporting macrophages to foster nerve regeneration after a peripheral nerve injury.
