Examines the toxicity of the chemical warfare agent sulphur mustard, bis-(2-chloroethyl)sulphide, in adult rat lung organ cultures. Investigators exposed lung tissue samples to the agent for 60 minutes and collected lung slices at specified time-points after exposure (2, 5, 8, and 21 days). They measured toxicity using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) cytotoxicity assay and histopathology analysis. Results presented include degree of lung damage versus expression times, the relationship between structural changes in lung tissue and biochemically measured toxicity, and degrees of similarity of the observed damage to the damage found in animals and man exposed to sulphur mustard in vivo. The results also indicate the usefulness of the in-vitro model for studying the toxicity and mechanism of action of sulphur mustard.

Thousands of Iranian people were injured by mustard gas in the Iraq war. This injury results in chronic disabilities of eyes, lung and skin organs. Chronic cough, dyspnea and hemoptysis were the major presenting symptoms in these patients. We studied late pulmonary complications of these patients. One hundred mustard gas victims were selected through a cross sectional study. All selected had documented criteria, pulmonary function tests, high resolution chest CT scan, bronchioscopy and routine blood tests, which were done in a well equipped center. All patients had chronic bronchitis in their bronchial biopsy. Pulmonary fibroses, with different types of histology, were detected in 80% of patients. A cytologic study of bronchial lavage did not show neoplastic cells. HRCT findings were compatible with bronchial thickening and subpleural fibrosis with definite correlation with histologic data in all patients. Late complications of mustard gas includes bronchial and paranchyiual involvement. Although the causative agent is not present, the disease has a continuous nature and sometimes progressive course, with end-stage lung disease the outcome.

Inhalation exposure to sulfur mustard (SM) vapor causes long term damage to the respiratory system. The lesions were characterized by specific physiological, biochemical and histopathological methods. Awake 128 guinea-pigs (GP) were exposed for 10 min to SM (1200-1700 microns x min/1). Respiratory parameters were monitored per animal before, during and after the exposure using plethysmograph's. Biochemical and histological evaluations were performed at different time intervals for up to 7 days post exposure. SM inhalation resulted in a decrease in both respiratory rate and minute volume, and in an increase in tidal volume. These changes occurred immediately after the onset of exposure and lasted for up to 7 days. The changes in the respiratory parameters were accompanied by a massive reduction in O2 diffusion capacity. Evaluation of bronchoalveolar lavage (BAL) fluid, indicated neutrophil infiltration, an increase in the protein content, and in the activity of both lysosomal enzymes and lactic dehydrogenase (LDH) in the alveolar space. In addition, a decrease in glutathione content was observed one day post exposure in the BAL fluid and the lung whereas an increase in lung glutathione content was observed 6 days later. Histological evaluation of the lungs and trachea revealed severe lesions in both tissues. Recovery was incomplete 7 days post exposure. The detailed characterization of the effect of SM inhalation offers a reliable model for the evaluation of potential therapies against SM exposure.

The Mustards Consortium has utilized both in vivo and in vitro models simultaneously to continue to elucidate mustard gas pathophysiology. In previous work done by the MC it was found that CEES, the mustard analogue, induced oxidative stress and was its primary mechanism of action. Consequently, NAC (N-acetyl cystiene) was found to be protective as a prophylaxis and treatment. A combination of a water and fat soluble antioxidant encapsulated in a liposome (STIMAL) was found to have the best ameliorative effect against CEES. We have initiated development of next generation STIMAL, in order to optimize its ameliorative effect. The mechanism of action of the antioxidants is suspected to be primarily by their effect on redox regulated pathways. In an effort to elucidate the mechanism of action of the antioxidants and the pathophysiology of mustards, profiles are being developed for: gene expression and antioxidant levels, as well as biochemical pathways. The first known histological comparison between CEES and sulfur mustard was carried out. Two new rat lung models were developed for the administration of sulfur mustards in preparation for efficacy testing of STIMAL against sulfur mustards. Pulmonary fibrosis was demonstrated in both guinea pig and rat lung models.

Inflammation and edema are associated with respiratory and cutaneous exposure to sulfur mustard (SM) as well as with phosgene-induced lung injury. IL-8 is a key inflammatory cytokine that recruits neutrophils linked with the onset and progression of acute lung injury caused by inhalation of these chemical agents. In the present study, human lung small airway cell (SAC) cultures were exposed to either SM [25 to 400 micrometers] or phosgene [0.1 to 6.4 ppm x min]. IL-8 was increased after exposure to either SM or phosgene. At the optimum exposures for SM (100 micrometers) and phosgene (1.6 ppm x min), IL-8 was increased by 1013 plus or minus 123 pg/ml and 965 plus or minus 181 pg/ml, respectively. Higher exposures to either agent increased cytotoxicity and decreased IL-8 levels. Ibuprofen has shown efficacy against phosgene pulmonary toxicity in mice. Ibuprofen (62, 125, 250, 500, 1000 micrometers) significantly diminished phosgene-increased IL-8 in SAC cultures exposed to 2 ppm x min phosgene. Maximum inhibition of nearly 50% of phosgene-increased IL-8 was seen at 125 and 250 micrometers doses of ibuprofen (from 1141 plus or minus 143 pg/ml to 628 plus or minus 105, 593 plus or minus 69 pg/ml respectively). Chemical insult-increased IL-8 in SAC cultures provides an assay for screening countermeasures against the inhalation toxicity of chemical threat agents. The increase in the inflammatory cytokine IL-8 by both SM and phosgene may further provide common pharmacological targets for drugs with Multi-Threat Medical Countermeasure (MTMC) action against distinct chemical threats.
