Background and aimChildren with Growth Hormone Deficiency (GHD) should have a brain Magnetic Resonance Imaging (MRI) scan performed as standard of care to evaluate the anatomy of the pituitary gland and brain .The aim of this study is to examine the relationship between the diagnosis of isolated GHD and brain MRI results in children who failed GH tests. Methods: A Retrospective chart review of all isolated GHD diagnoses and their brain MRI findings was performed.A total of 120 children (85 males, 35 females) with GHD were identified. Age range was between 3 to 16 years. The diagnosis of GHD was based on levels below 10 ng/ml ( 10 u00b5g/L) after a stimulation tests with 2 agents ( Arginine, Clonidine) .Cases with pre-existing conditions ( cancer, trauma) , panhypopityitarism or known brain anomalies prior to the diagnosis of GHD were excluded.Results: A normal MRI of the brain was the most common finding (79%) followed by Arnold Chiari malformation (10%). A small pituitary gland was noted in 8% of the cases. The rest of the MRI findings included of brain AV malformation, craniopharyngioma and a brain tumor (a total of 3%, one case each).Conclusions: Children with GHD should get an MRI scan for elucidating the underlying etiology. Growth hormone deficiency may be a result of congenital brain malformation, poor pituitary development or tumor. Short stature may be the only initial feature. Finding an intracranial pathology may be a contraindication for the immediate use of GH therapy.

PurposeA wide spectrum of pathology can involve the clivus from metastatic tumors and invasive tumors, to primary and hematopoietic neoplasm. Given clinical findings are nonspecific, if present, imaging is required for identifying and characterizing lesions affecting this region. However, given the breadth of pathology involving the clivus, classifying the lesion can be challenging, requiring localization of the center of the mass and knowledge of signal characteristics of potential pathology. In this pictorial review, we will discuss the normal anatomy of the clivus and surrounding structures, along with key differentiating imaging features of various pathological conditions involving the clivus. Approach/MethodsUsing diagrams, MRI, and CT we will discuss the anatomy of the clivus, particularly its relationship to surrounding anatomic structures. Using representative case files, we will review a variety of common and uncommon pathologies affecting the clivus including: chondrosarcoma (Fig 1D), chordoma (Fig 1C), giant cell reparative granuloma, clival metastasis, fibrous dysplasia, multiple myeloma, plasmacytoma (Fig 1B), lymphoma, intraosseous meningioma, ecchordosis physaliphora, invasive pituitary macroadenoma (Fig 1A), sphenoid sinus mucocele, invasive nasopharyngeal carcinoma. We will discuss imaging features that can help in differentiating the pathology within these spaces and in formulating an appropriate differential diagnosis. ResultsKey Learning Points:Differential diagnoses and features of pathology of the clivus using a case-based approach, including their imaging appearance.Normal anatomy of the clivus and surrounding structures will also be reviewed.ConclusionEvaluating clival pathology on imaging requires an understanding of the surrounding anatomy and pathology involving those structures, in addition to intrinsic disease processes. Attending this exhibit will provide viewers with an in-depth pictorial review of both common and uncommon pathologies affecting the clivus, differential diagnoses, and how to differentiate lesions involving this region on imaging with emphasis on key differentiating imaging features.
