The Naval Research Laboratory (NRL), at the request of the U.S. Army Engineer Power Group (USAENPG), has performed a failure analysis on two MH-1A reactor core hold-down bolts that broke in service. Adherence to fabrication specifications, post-service properties and possible causes of bolt failure were investigated. The bolt material was verified as 17-4PH precipitation hardening stainless steel. Measured bolt dimensions also were in accordance with fabrication drawing specifications. Bolt failure occurred in the region of a locking pin hole which reduced the bolt net section by 47 percent. The failure analysis indicates that the probable cause of failure was net section overloading resulting from a lateral bending force on the bolt. The analysis indicates that net section overloading could also have resulted from combined tensile stresses (bolt preloading plus differential thermal expansion). Recommendations are made for improved bolting.

This text is intended for use in a one-semester, undergraduate course in metallurgy. The goal is to provide simple but practical explanations of the scientific principles which govern metallurgy through the use of abundant illustrations. Beginning with a concise review of key concepts, the first section covers the atom's structure (the planetary model through cursory concepts of quantum mechanics), the arrangement of the periodic table of elements, and concludes with an explanation of atomic bonding. The second section examines the three primary metallic crystals, Miller/Miller-Bravais directions and indices, crystalline imperfections, and the important topic of phase diagrams. Mechanical metallurgy leads off the final section, followed by ferrous metallurgy, before ending with a brief overview of electrochemical corrosion. Also included in this revised edition is a descriptive explanation of thermodynamics and an optional section on statistical quality control. And yet, while many formulas are presented, only a basic understanding of algebra is required; just as the more involved concepts of chemistry and thermodynamics have been kept to a minimum. J .W. Bell did undergraduate studies in Applied Science (NDE) and graduate studies in Operations Research and System Safety Engineering. Currently he works in nondestructive evaluation as a metallurgical inspector and analyst.

A metallurgical analysis has been performed upon a failed control rod cap from the Navy PM-3A Nuclear Power Plant. Material supplied to the Metallurgy Division for this investigation was the lifting knob which had separated in service from the main body of the cap. The specific objective of the analysis was to determine if the material of application met design specifications and to determine the probable cause of component failure. Compositional analysis by x-ray spectrochemical and gravimetric techniques established that the lifting knob, and thus the failed control rod cap, was fabricated from a 300 series stainless steel, not from 17-4 PH precipitation hardening stainless steel as specified. Microstructural and hardness examinations further confirmed the substitution of a 300 series stainless steel for the 17-4 PH steel composition. Examination of the stem of the control rod cap knob at which failure occurred revealed severe 'necking down' and heavy striations corresponding to work hardening lines. This visual evidence coupled with analytical evidence of a low strength material of application suggests component failure through gross overload. (Author).
