Mechanisms of brain-immune interactions became a cutting-edge topic in systemic neurosciences over the past years. Acute lesions of the brain parenchyma, particularly, induce a profound and highly complex neuroinflammatory reaction with similar mechanistic properties between differing disease paradigms like ischemic stroke, intracerebral hemorrhage (ICH) and traumatic brain injury (TBI). Resident microglial cells sense tissue damage and initiate inflammation, activation of the endothelial brain-immune interface promotes recruitment of systemic immune cells to the brain and systemic humoral immune mediators (e.g. complements and cytokines) enter the brain through the damaged blood-brain barrier. These cellular and humoral constituents of the neuroinflammatory reaction to brain injury contribute substantially to secondary brain damage and neurodegeneration. Diverse inflammatory cascades such as pro-inflammatory cytokine secretion of invading leukocytes and direct cell-cell-contact cytotoxicity between lymphocytes and neurons have been demonstrated to mediate the inflammatory ‘collateral damage’ in models of acute brain injury. Besides mediating neuronal cell loss and degeneration, secondary inflammatory mechanisms also contribute to functional modulation of neurons and the impact of post-lesional neuroinflammation can even be detected on the behavioral level. The contribution of several specific immune cell subpopulations to the complex orchestration of secondary neuroinflammation has been revealed just recently. However, the differential vulnerability of specific neuronal cell types and the molecular mechanisms of inflammatory neurodegeneration are still elusive. Furthermore, we are only on the verge of characterizing the control of long-term recovery and neuronal plasticity after brain damage by inflammatory pathways. Yet, a more detailed but also comprehensive understanding of the multifaceted interaction of these two supersystems is of direct translational relevance. Immunotherapeutic strategies currently shift to the center of translational research in acute CNS lesion since all clinical trials investigating direct neuroprotective therapies failed. To advance our knowledge on brain-immune communications after brain damage an interdisciplinary approach covered by cellular neuroscience as well as neuroimmunology, brain imaging and behavioral sciences is crucial to thoroughly depict the intricate mechanisms.

Neuroinflammation is a burgeoning area of interest in academia and biopharma, with a broadly acknowledged role in many central nervous system (CNS) disorders. To address these gaps in understanding mechanisms and how to translate that understanding into therapeutics, the Forum on Neuroscience and Nervous System Disorders of the National Academies of Sciences, Engineering, and Medicine convened a workshop on March 20-21, 2017, bringing together key leaders in the field from industry, academia, and governmental agencies to explore the role and mechanisms of neuroinflammation in a variety of CNS diseases. The workshop also considered strategies to advance the identification and validation of biomarkers of neuroinflammation that could accelerate development of therapies, bringing much-needed treatments to patients with disorders ranging from neuroinflammatory diseases such as multiple sclerosis (MS) to neuropsychiatric disorders such as depression. Traumatic Brain Injury (TBI) often results in prolonged or permanent brain dysfunction with over 2.8 million affected annually in the U.S., including over 56,000 deaths, with over 5 million total survivors exhibiting chronic deficits. Mild TBI (also known as concussion) accounts for over 75% of all TBIs every year. Mild TBI is a heterogeneous disorder, and long-term outcomes are dependent on the type and severity of the initial physical event and compounded by secondary pathophysiological consequences such as neuroinflammation, reactive astrocytosis, edema, hypoxia, and excitotoxicity. Neuroinflammation has gained increasing attention for its role in secondary TBI injury as it can have both detrimental and beneficial roles. For example, microglia - the resident immune cell of the central nervous system (CNS) - mediate cell death and may contribute to progressive neurodegeneration but also aid in debris clearance and regeneration. However, prior to the following work microglia had not been studied after diffuse mild TBI in a clinically relevant large animal model at chronic time points.Replicating the injury biomechanics of human mild TBI in animal models presents a substantial challenge, particularly with regards to addressing brain size, neuroanatomy, and acceleration parameters. To address this challenge, our lab uses a swine model of diffuse closed-head TBI that accurately replicates human mild TBI mechanisms and manifestations due to the neuroanatomical similarities between humans and pigs (large gyrencephalic brains with high white to grey matter ratios) that are not present in small animals. This well-characterized model subjects the head to rapid angular acceleration, inducing inertial forces common in human TBI resulting from falls, collisions, or blunt impacts. This injury model has been shown to produce acute and chronic pathologies in patterns that mirror those seen in humans.Therefore, using this rotation-acceleration model of closed-head TBI in pigs, the goal of the studies reported herein was to elucidate the role of microglia and the neuroinflammatory response to single mild TBI out to 1 year post injury through in-depth histological profiling in discrete neuroanatomical regions across three specific aims. First, we assessed the relationship between the hallmark axonal pathology of diffuse TBI and alterations in glial cell activation or morphology. Then, we examined neuroinflammatory and synaptic changes in sub-regions of the dentate gyrus following a single mild TBI, as the hippocampus is notably susceptible to damage post-TBI and microglia may play a role in synaptic remodeling. Finally, we assessed gray matter neuropathology and neuroinflammation in response to a novel mechanism of immediate neuronal membrane disruption following TBI. These studies will inform future work to characterize the underlying molecular and neurophysiological alterations post-TBI, as well as potential contributions to neurological deficits. It is hoped that these aims will translate to advances in the treatment and prevention of the long-term consequences of TBI in people.

Traumatic brain injury (TBI) is a major public health problem as it is a major cause of death and disability in all age groups. The pathophysiology of TBI is complex and heterogeneous. The primary injury at the time of trauma initiates a chain of events called secondary injury that extends beyond the initial site of injury and leads to additional neurological damage and impairments in cognitive function. TBI is an important risk factor for dementia, and accumulating evidence indicates that mild TBI (mTBI) can result in long-lasting pathological changes associated with neurologic and behavioral disabilities. However, the cellular mechanisms underlying these changes are largely unknown. The central hypothesis of the thesis is that leukotrienes, pro-inflammatory lipid mediators, activate neuroinflammatory signaling cascades, which ultimately contribute to detrimental secondary injury and associated cognitive deficits. First, I investigated the role of leukotrienes in moderate to severe TBI using the fluid percussion injury model and the efficacy of blocking their production with a FLAP inhibitor. Administration of a FLAP inhibitor shortly after TBI significantly blocked injury-induced leukotriene synthesis, attenuated brain edema and blood-brain barrier disruption, and protected against impairments in synaptic plasticity and memory and learning. Next, I investigated the spatiotemporal nature of secondary injury and neuroinflammation after mTBI using a newly developed closed head injury (CHI) model. CHI resulted in a robust neuroinflammatory response that was initiated early and persisted for at least one month in discrete brain regions. This neuroinflammatory response occurred in the absence of macroscopic lesions and blood-brain barrier disruption and was accompanied by neuronal and axonal degeneration. Acute administration of a FLAP inhibitor significantly blocked both acute and chronic neuroinflammation and the corresponding neurodegeneration. Last, I used a novel, unbiased proteomic approach to screen plasma at several time points after CHI and identified 7 putative biomarkers of mTBI. Taken together, these results indicate that the 5-lipoxygenase pathway of leukotriene biosynthesis initiates and maintains an endogenous immune response to brain injury that leads to delayed neurodegeneration and cognitive impairments. These findings have important implications for treating human TBI and provide support for FLAP inhibitors as a novel, targeted intervention.

Neurofibrillary tangles (NFTs) composed of intracellular aggregates of tau protein are a key neuropathological feature of Alzheimer’s Disease (AD) and other neurodegenerative diseases, collectively termed tauopathies. The abundance of NFTs has been reported to correlate positively with the severity of cognitive impairment in AD. However, accumulating evidences derived from studies of experimental models have identified that NFTs themselves may not be neurotoxic. Now, many of tau researchers are seeking a “toxic” form of tau protein. Moreover, it was suggested that a “toxic” tau was capable to seed aggregation of native tau protein and to propagate in a prion-like manner. However, the exact neurotoxic tau species remain unclear. Because mature tangles seem to be non-toxic component, “tau oligomers” as the candidate of “toxic” tau have been investigated for more than one decade. In this topic, we will discuss our consensus of “tau oligomers” because the term of “tau oligomers” [e.g. dimer (disulfide bond-dependent or independent), multimer (more than dimer), granular (definition by EM or AFM) and maybe small filamentous aggregates] has been used by each researchers definition. From a biochemical point of view, tau protein has several unique characteristics such as natively unfolded conformation, thermo-stability, acid-stability, and capability of post-translational modifications. Although tau protein research has been continued for a long time, we are still missing the mechanisms of NFT formation. It is unclear how the conversion is occurred from natively unfolded protein to abnormally mis-folded protein. It remains unknown how tau protein can be formed filaments [e.g. paired helical filament (PHF), straight filament and twisted filament] in cells albeit in vitro studies confirmed tau self-assembly by several inducing factors. Researchers are still debating whether tau oligomerization is primary event rather than tau phosphorylation in the tau pathogenesis. Inhibition of either tau phosphorylation or aggregation has been investigated for the prevention of tauopathies, however, it will make an irrelevant result if we don’t know an exact target of neurotoxicity. It is a time to have a consensus of definition, terminology and methodology for the identification of “tau oligomers”.
