Book Description :

The development of flap surgery parallels the increasing complexity of soft-tissue defects needing reconstruction. Random and pedicled flaps as well as free muscle and fasciocutaneous flaps have helped to reconstruct single soft-tissue defects. The multiplicity of defects needing reconstruction and donor-site morbidity in addition to tailored reconstruction have called for a revision of flap concepts in favor of perforator flaps. Unfortunately, we are faced with increasingly complex reconstructive issues. New reconstructive techniques, such as the Ilizarov method, have made orthopedic reconstruction after high energy and complex trauma possible. Revision surgeries after tumor resection and plastic surgery have brought about soft-tissue defects associated with extensive fibrosis and necrosis. As a result, previously nonsalvageable limbs have been salvaged. The reconstructive surgeons are faced with the following situations: multiple soft-tissue defects, extensive fibrosis, possibility of major vessel loss, and possibility of damage of several perforators.

Book Description :

This book was written for orthopedic and plastic surgeons who wish to learn about reconstructive surgery of the extremities and trunk. For reconstructions of the extremities and trunk, surgeons need information on both orthopedic and plastic surgery. However, orthopedic surgeons are generally not familiar with the reconstruction of soft tissue, and plastic surgeons have little experience with osseo-tendinous system reconstructions. For those reasons, there is a definite need for a book detailing both fields of surgery. Because many existing books focus on only the type of flap, it is difficult for doctors to distinguish which flap is best for their patients in need of reconstructive surgery. In this book, chapters are based not on the flap but the injury site. In the each chapter, pitfalls and candidate flaps are described for the reconstruction of each site. Additionally, the operative steps are explained with the help of numerous high-quality photos, offering a unique valuable resource for orthopedic and plastic surgeons faced with these challenges.

Book Description :

The use of perforator flaps has heralded a new era in reconstructive surgery. Perforator flaps are less bulky than traditional flaps and do not use muscle tissue.? These characteristics allow reconstructions using these flaps to achieve the best functional and aesthetic results in both the reconstruction site and the donor site. This issue of Clinics in Plastic Surgery provides guidance to help plastic surgeons incorporate perforator flap techniques in their practices. Editor Peter Neligan, one of the world's leading experts in perforator flaps, has assembled highly experienced authors to share their expertise with Clinics readers.

The book begins with a chapter on basic principles, which reviews the important oncologic concerns that must precede any reconstructive-surgery procedure. Principles of tissue movement and flap design are accompanied by drawings that aid the reader in planning. The next seven chapters address each anatomic region. Alternate methods for closing defects in each region are presented. Both simple and complex solutions are detailed. Each reconstructive option is presented in great detail with operative photos as well as beautiful illustrations to depict flap design, elevation, and closure, and to more clearly explicate complex flap reconstructions for difficult problems. Common mistakes and the limitations of each flap are clearly delineated.
