Background Cerebral small vessel disease (CSVD) may be part of a multisystemic small vessel disorder also affecting the kidneys, but most previous data on this topic was retrospective and used limited renal parameters. In this prospective study of stroke patients with CSVD-related recent small subcortical infarction (RSSI), we hypothesized that markers of renal dysfunction were associated with the burden of CSVD on baseline MRI and its progression at follow-up MRI.MethodsWe prospectively investigated 101 patients (mean age: 60u00b111 years, 73% men) with MRI-confirmed RSSI who underwent follow-ups until 15 months poststroke. Renal parameters assessed included eGFR, Albumin-Creatinine Ratio and fibroblast growth factor-23 (FGF-23, an early marker of renal dysfunction). Chronic kidney disease (CKD) was defined according to current guidelines. Baseline CSVD burden and progression were assessed on brain MRI. ResultsRSSI patients with CKD (n=24, 24%) had more severe WMH at baseline (Fazekas 2-3 in 58% vs. 36%, p=0.04), other MRI markers of chronic CSVD did not differ compared to patients without CKD. CKD did not predict CSVD progression at 15 months. However, patients in the highest FGF-23 quartile more frequently showed CSVD progression (50% vs. 21%, OR 3.89, p=0.03) and showed increased rates of recurrent vascular events in the observation period (53% vs. 19%, OR 4.93, p=0.003). ConclusionsAlthough the severity of WMH was associated with CKD, conventional parameters of renal dysfunction did not predict progression of CSVD. However, our data suggest FGF-23 as a new biomarker of CSVD progression and recurrence of vascular events in RSSI patients, which warrants further studies.

The fibroblast growth factor (FGF) family of ligands and the corresponding family of FGF receptor tyrosine kinases comprise one of the most versatile and diverse growth factor signaling families in vertebrates, found virtually in every tissue and cell type where they regulate metabolic and physiologic function, maintain tissue homeostasis, and mediate injury response, tissue repair, and regeneration. As a result, FGFs play critical roles in a wide variety of normal biological and abnormalities in FGF signaling can lead to a variety of disease states from developmental malformations, to atherosclerosis, pulmonary hypertension and cancer among many others. A thorough understanding of this system is necessary for critical insights into both normal and disease biology and is essential to development of rationale therapeutic strategies aimed at treatment of FGF-dependent diseases states. Recent years have seen advances in our understanding of FGFs role in endothelial and, more general, vascular biology, the subject of this book. In particular, FGFs have been implicated in the maintenance of vascular homeostasis and control of blood vessel permeability while aberrant FGF signaling has been shown to be central to the development of pulmonary hypertension, and endothelial-to-mesenchymal transition, a process implicated in the development of atherosclerosis. Finally, FGFs ability to stimulate blood vessel growth has been explored in therapeutic angiogenesis approaches. With this in mind, the present monograph is structured to provide comprehensive information to a reader interested in FGFs in general and vascular biology in particular. To this end, we explore everything from the structure and signaling of FGFs and their receptors (Chapters 1, 2 and 3) to their role in the regulation of vascular homeostasis (Chapter 4) and cardiovascular diseases (Chapters 5 and 6). Abstract : Abstract: In chronic kidney disease (CKD), decreased erythropoietin production, low serum active vitamin D levels, and high renin-angiotensin-aldosterone activities had been regarded as major causes of renal anemia. At present, no clinical data are available to elucidate the association between renal anemia and fibroblast growth factor 23 (FGF23) levels in CKD. This study aimed to access whether FGF23 is involved in the pathogenesis of renal anemia. This cross-sectional observational study included 53 stable outpatients with CKD stages 3 and 4. Our primary predictor was serum FGF23 levels and outcome was hemoglobin levels. Measurements contained hemoglobin, FGF23, 25-hydroxyvitamin D, intact parathyroid hormone, plasma renin, serum aldosterone, HbA1C levels, lipid and iron profiles, and serum and urine electrolytes. Mean age of our patients was 66.4 ± 12.8 (SD) years, mean estimated glomerular filtration rate 33.5 ± 13.9 mL/min/1.73 m 2, median FGF23 level 200 (25th–75th percentile, 124–303) pg/mL, vitamin D level 19.5 (25th–75th percentile, 14.0–25.9) ng/mL, and hemoglobin level 12.7 (25th–75th percentile, 10.7–13.75) g/dL. Even after adjusting multiple variables, lower hemoglobin levels correlated significantly with FGF23 levels that were higher than the median value (>200 pg/mL). Moreover, after adjusting for aldosterone, but not 25-hydroxyvitamin D, it decreased the association with FGF23 that higher than median level and hemoglobin levels. We also observed a significant decrease of hemoglobin level in the higher FGF23 group who had a diabetes history. High FGF23 levels were observed to be associated with low hemoglobin levels, which may be partially mediated through the effects of serum aldosterone levels in our patients with CKD stages 3 and 4. Furthermore, we also presumed that diabetes itself may have an impact on the loop among FGF23, hemoglobin, and aldosterone levels in these CKD patients.

Abstract: Fibroblast growth factor-23 (FGF23) is a bone-derived hormone known to suppress phosphate reabsorption and vitamin D hormone production in the kidney. Klotho was originally discovered as an anti-aging factor, but the functional role of Klotho is still a controversial issue. Three major functions have been proposed, a hormonal function of soluble Klotho, an enzymatic function as glycosidase, and the function as an obligatory co-receptor for FGF23 signaling. The purpose of this review is to highlight the recent advances in the area of FGF23 and Klotho signaling in the kidney, in the parathyroid gland, in the cardiovascular system, in bone, and in the central nervous system. During recent years, major new functions of FGF23 and Klotho have been discovered in these organ systems. Based on these novel findings, FGF23 has emerged as a pleiotropic endocrine and auto-/paracrine factor influencing not only mineral metabolism but also cardiovascular function.
