ABSTRACT: In the USA today, running is a very popular recreational activity. With the majority of runners being female, it is important to understand how physiological changes during and after pregnancy may affect a woman’s return to running. 1 Over the duration of pregnancy, a woman’s body undergoes several physiological changes that can last several months after delivery. 2 In 2018, the American College of Obstetricians and Gynecologists (ACOG) updated guidelines which introduced a fourth trimester revolving around the care of females directly following delivery. 3 This suggests that postpartum care should be addressed on multiple occasions following childbirth. While weight gain and lower extremity edema may be the most noticeable changes during pregnancy, almost every system is affected. 4 Changes that occur range from ligament laxity and muscular weakness, to cardiovascular and pulmonary decline. 4 Specifically, the pelvic floor muscles (PFM) undergo significant modifications including stretch weakness from the increased weight gain, and possible tearing during childbirth. 2 These changes may cause decreased trunk stability, pelvic floor weakness, and urinary incontinence, especially with strenuous activities such as running. 2 When running, intra-abdominal pressure (IAP) naturally increases from ground reaction forces acting on the body. 2 The abdominal musculature and PF act to prevent IAP from further increasing. If an individual has weakened PFM, urinary incontinence and altered mechanics may ensue when running. 2 Analysis indicates that women’s participation in exercise programs severely decreases after childbirth. 5 Although the postpartum period is an opportunity for women to return to exercise, most have been recommended to avoid exercise until at least six weeks postpartum. 5–7 Exercise is defined by the American Congress of Obstetricians and Gynecologists as 30 minutes or more of moderate intensity per day, and women should participate most or all days. 8 Exercise consists of both aerobic cardiovascular training and musculoskeletal resistance training. 5 Recommended exercises during the immediate postpartum period involve restoring strength and function of the pelvic floor muscles. 6 This may include performing Kegel exercises and various abdominal contraction exercises. 9 Kegels are isolated pelvic floor muscle contractions that are held for a few seconds. 9 Research states the most prominent risk factors for injury while running include history of previous injury, being a novice, and overtraining. 2 Women who begin or return to running for the first time postpartum are considered novice. This is due to recent physiological and physical changes in their bodies, lack of sleep, fatigue of having a newborn, and physical recovery after childbirth, all of which puts them at a higher risk of injury. 2 Currently, there is little evidence for women in the postpartum period regarding intensity, duration, frequency, or pre-running strengthening activities. 10 To date there are no national or international guidelines for postpartum women to follow when initiating or returning to running. 10 It is paramount to provide women with evidence-based recommendations regarding a safe return to exercise following childbirth. Therefore, the following manuscript reviews evidence-based activities and protocols for women to return to running during the postpartum period.
