Book Description :

What is evidence-based health communication and how does it benefit patients? How has communication been taught to health professionals and how might it be improved? How can health care professionals make the most of short encounters in order to maximise their therapeutic effectiveness for clients? This book provides a comprehensive and critical review of the field of health communication and the kinds of evidence that have been collected concerning effective communication. It also critically evaluates the kinds of training health professionals receive in communication skills and examines its relatively limited role in the curriculum. In addition it sets out what has been discovered about the micro-structure of interaction in health care encounters. The book offers vital new agendas for research training and practice in health care, based upon lessons learned from linguistics, using a wide variety of gathered evidence to identify patterns that will lead to improved health care practices. Moreover, the book focuses upon brief, ordinary and effective communicative activity in addition to the formal consultations that have been studied by researchers in the past. Evidence-based Health Communicationis key reading for trainee health professionals as well as undergraduate and postgraduate students in the fields of health studies, medical sociology and health psychology. It also provides stimulating reading for health care professionals, policy makers and researchers with an interest in improving health communication.

Book Description :

The capacity of professional medicine to resist change - and also concordance - is impressive, but perplexing. It is one of the issues I seek to address in this book. I suggest that a preoccupation with trying to change the relationship between the professional-patient dyad has deflected attention from the extent to which such relations are embedded in, and constrained by, wider administrative and organisational structures, especially as these relate to the operation of professional hierarchies and interprofessional deference and allegiances. Barriers to change also result from the inertia of a system which has evolved a highly stylised etiquette as an adaptive mechanism to contain the difficulties and tensions intrinsic to the medical consultation. Its therapeutic purpose and potential are often subordinate to the goal of achieving success as a social encounter. The principles of concordance are deeply challenging to traditional professional roles and status. However, medicine has always displayed an ability to block change through tactics of appropriation and incorporation. Professionals have often shown particular difficulty giving up their monopoly of 'expertise' and in acknowledging the legitimacy of the patient perspective. Although the term 'concordance' has become quite widely used, its meaning is usually subverted by its employment as a synonym for 'compliance', albeit 'informed' compliance. A slightly more sophisticated version values professional elicitation of the patient perspective in order to more accurately tailor information as a means of overcoming the unhelpful m/sconceptions that impede compliance. The original emphasis on the consultation as a negotiated exchange, in which the professional has something of value to learn from the patient, has largely been lost. The rhetoric of modernity and change provides an effective mask for inertia and conservatism. Preface.

Book Description :

"Hospital-Based Palliative Medicine: A Practical, Evidence-Based Approach focuses on improving a Hospitalist's ability to provide quality palliative care in the inpatient hospital setting. Although several large textbooks exist for the palliative care provider, there are few detailed resources aimed specifically at the general Hospitalist and hospital-based staff. Existing textbooks dedicate at most only one chapter to palliative care and they lack the detail and specificity in symptom management and communication skills to truly guide care. Hospital-Based Palliative Medicine addresses each of the major components involved in the delivery of inpatient palliative care. These include the management of complex symptoms and the use of specific communication techniques - right down to the actual words to say to the patient - to convey important information such as the reason for and use of medication and how to handle difficult topics with patients and their families and caregivers. The book's authors are all leaders in the field of palliative care, and most of them also practice as hospitalists, and thus understand both perspectives. Chapters are presented in a consistent format with particular attention to both clinical pearls and pitfalls, making the material practical and easily accessible to the busy hospitalist and associated hospital staff"--Provided by publisher.

Designed to support the widely recognised PARIHS framework, which works as a guide to plan, action and evaluate the implementation of evidence into practice, this book provides a very practical ‘how-to’ guide for facilitating the whole process. This text discusses: undertaking an initial diagnosis of the context and reaching a consensus on the evidence to be implemented; how to link the research evidence with clinical and patients’ experience and local information in the form of audit data or patient and staff feedback; the range of diagnostic, consensus building and stakeholder consultation methods that can be helpful; a description of facilitator roles and facilitation methods, tools and techniques; some of theories that underpin the PARIHS framework and how these have been integrated to inform a revised version of PARIHS Including internationally-sourced case study examples to illustrate how the facilitation role and facilitation skills have been applied in a range of different health care settings, this is the ideal text for those interested in leading or facilitating evidence based implementation projects, from the planning stage through to evaluation.
