Despite recent advances in the detection, diagnosis and treatment of cancer, this has not translated into increased survival, with the overall 5-year survival only modestly improved in the last 15 years. For patients with cancers unresponsive to conventional intervention, development of alternative therapies is crucial. Chemotherapy involves systemic delivery of cytotoxic drugs and has significant side effects. Electrochemotherapy is a new technique using electric pulses applied to tumour tissue rendering the tumour cell membranes permeable to otherwise impermeant or poorly permeant cytocidal drugs, thereby facilitating a potent local cytotoxic effect. We have used this in a patient population who had cutaneous or subcutaneous tumour recurrences and had exhausted current cancer therapies. We have found it to be a safe and effective localised therapy for surface metastatic nodules, recalcitrant to conventional oncologic therapies in these patients. Using our experience in combination with 3 other centres we have defined optimal analgesic protocols, electrode selection and selection of chemotherapeutic drug and delivery routes for this novel delivery system. Cancer is often a systemic disease at presentation and while localised tumour therapies such as electrochemotherapy would significantly reduce the cytotoxic effects of the chemotherapeutic drugs, minimal residual disease following surgical excision or drug therapy requires alternative treatments. Gene therapy of overt cancers may enhance a specific host anti-tumour response, by induction of cytokine or co-stimulatory immune molecule production by the tumour cell. Delivery of these genes in plasmid constructs to tumours by electropermeabilisation allows localised expression with high transfection efficiency without the side effects associated with other delivery methods such as viruses. Using a GMCSF/B7-1 combination plasmid delivered by electroporation into murine solid tumours we have induced effective local and systemic anti-tumoral immune responses, which are durable. This therapy was successful in eradication of the primary tumour in more than 60% of cases. Such a strategy could be used to produce a patient specific cancer therapy without requiring ex-vivo manipulation of the tumour cells. Further modification of the delivery system as well as using this as adjuvant therapy followed by excision of the primary tumour may allow further improvements in this gene based therapy. Electroporation-based therapies have been limited to surface tumours thus far, application to internal tumours would expand the therapeutic applications. We have designed and tested novel electrode designs for laparoscopic and endoscopic deployment for electroporation-based therapies. We have shown functionality of these electrode designs which will allow access to heretofore-inaccessible tumours.

Most physical ablation modalities for cancer treatment are focal and are based on thermal damage. Despite their regular clinical use as an alternative to surgical resection, their thermal principle of operation entails risks regarding the preservation of neighboring vital structures, such as large vessels, critical ducts or nerves. In addition, being focal, their use is unpractical in cases where multiple nodules are present or tumors are difficult to reach with the applicators. This thesis explores non-thermal electrical treatments which can be applied in a non-focal manner. Two treatments have been investigated: the first treatment, proposed by others a few years ago, is based on the permanent application of low magnitude alternating electric fields through surface electrodes. Here, this treatment has been in vivo studied to evaluate its efficacy as well as to discern whether it is non-thermally mediated. The second electrical treatment is based on the electroporation phenomenon and targets liver tumor nodules. Electroporation-based therapies employ brief high magnitude electric fields. These pulsed fields, alone or in combination with chemotherapeutic drugs, are able to kill cells by increasing their membrane permeability. Current electroporation-based therapies for internal tumors are local and are delivered through needle-shaped electrodes. Rather than using needle electrodes to treat liver tumors, here it is explored a novel treatment in which large plate electrodes are used to deliver the field across the whole liver in a non local fashion. The treatment aims at simultaneously destroying all tumors while preserving healthy tissue. Its efficacy is based on selectively enhancing the electric field over the tumors by infusing a solution with high electrical conductivity. The proposed treatment for liver tumors requires a high performance generator which is not currently available. The work presented here includes the design of a new generator topology able to fulfill the requirements.

Non-thermal irreversible electroporation is a new minimally invasive surgical p- cedure with unique molecular selectivity attributes – in fact it may be considered the first clinical molecular surgery procedure. Non-thermal irreversible electro- ration is a molecular selective mode of cell ablation that employs brief electrical fields to produce nanoscale defects in the cell membrane, which can lead to cell death, without an effect on any of the other tissue molecules.
