Book Description :

Gut microbiome has a profound effect on human health and disease. The burgeoning body of research investigating the gut microbiome has illustrated its important roles on not only the local inflammatory diseases, such as inflammatory bowel disease, but also on systemic immunological disorders, such as autoimmune diseases. Many of these associations were determined at the level of metagenome analysis. This provides categorical information on both taxonomic composition and predicted functional capacities of the microbial community. This thesis tackles the problem of how gut microbiota affect human diseases using both these categories of assessment. Although the accumulating evidence suggested the important roles of gut microbiota in the human immune system and Crohn's disease (CD), the molecular mechanisms of how microbiome directly modulates immune responses and leads to CD progression remain unclear. To advance the current understanding of microbial roles in CD, we developed a systems biology approach to assess the human immune function in response to disease-associated microbial products. Progress in computational omics analysis of the microbiome in CD enables the functional inference and prediction of microbial metabolites. Both genetic and clinical studies suggested the involvement of human CD4+ T cells in CD progression. We therefore tabulated a set of microbial metabolites predicted to be differentially abundant in CD, and screened these microbial metabolites for their bioactivity in human CD4+ T cell functions. Our screen revealed 15 bioactive microbial metabolites, 3 previously reported and 12 unprecedented, with selective action on CD4+ T cell cytokine production. Mechanistic assessment of one novel microbe-derived metabolite, ascorbate, revealed apoptosis of activated human CD4+ T cells associated with selective inhibition of glycolytic energy metabolism. These findings suggest a substantial rate (11%) of bioactive metabolites among the predicted CD-associated metabolite reservoirs and provide evidence for novel modes of microbial activity targeting T cell metabolism. Emerging studies have characterized the association of taxonomic composition with systemic autoimmune diseases with various clinical phenotypes and modes of pathologic immune responses. In this thesis, we assessed systemic sclerosis (SSc), a previously unstudied autoimmune disease with respect to the microbiome, notable for both systemic manifestations and gastrointestinal involvement. The metagenomic study of mucosal-luminal interface (MLI) samples revealed a unique microbial composition associated with disease state. Patients with SSc had decreased abundances of Faecalibacterium and Clostridium, and increased abundances of Bifidobacterium, Lactobacillus, Fusobacterium and -Proteobacteria compared with healthy controls. In addition, the increase of Fusobacterium species and the decrease of Bacteroides fragilis were associated with moderate/severe gastrointestinal symptoms. We conducted an independent study of faecal samples from two independent cohorts of patients, from United States and Norway, that revealed a similar group of microbial taxa associations, and a greater dysbiosis in patients from the United States. These studies enable the specific targets for intervention to avert or treat the gastrointestinal involvement in systemic sclerosis. The study of gut microbiota is beginning to evolve from association toward causality. The functions of gut microbiota are not only restricted to local inflammatory responses but expanded to systemic immune regulations.i We hypothesized that gut microbiome may regulate host immune cellular responses by producing metabolites. We investigated the bioactive microbial factors regulating Crohn's disease, and explored the potential microbial taxonomic biomarkers in a systemic autoimmune disorder, SSc. Those improved understandings of the microbiome activity and metabolism may inform therapeutic strategies and ultimately improve patient health.

Book Description :

Pediatrics in Systemic Autoimmune Diseases, Second Edition, provides a good starting point for physicians and scientists new to the field and for anyone looking for the most up-to-date summary of a particular disease. While many questions are answered, many more are raised for which we do not yet have the answers. The book provides inspiration to readers conducting their own research and helps to answer their questions. The care of children with systemic autoimmune diseases is moving forward at a rapid pace. The systemic autoimmune diseases of childhood have always represented a special problem for the physicians and scientists who care for the affected children and study their diseases. Some conditions, such as Kawasaki disease, are virtually unique to childhood, while systemic lupus erythematosus, progressive systemic sclerosis, and juvenile onset spondyloarthropathies may be thought of as the early onset of the same disease which is seen in adults. The autoinflammatory disorders appear to be of clear genetic origin, accounting for their frequent discovery in childhood. For many other conditions, such as juvenile dermatomyositis, the relationship between the conditions seen in children and that seen in adults is less certain. This book explores these phenomena, giving users a foundation on which to understand certain diseases. Includes completely updated chapters and five new chapters Explains the differences in systemic autoimmune diseases seen in children vs. those seen in adults Covers the latest advances in pathogenesis and clinical management of common conditions seen in pediatric rheumatology practices, benefiting both pediatric and adult rheumatologists, as well as physicians from other specialties Brings the reader up-to-date and allows easy access to individual topics in one place Provides the latest information available in a format that would give the clinician both a clear understanding of what has been done in the past and a scientific basis for deciding what to do for the next afflicted child in their care

Book Description :

Systemic autoimmune diseases can affect both the central and peripheral nervous systems in a myriad of ways and through a heterogeneous number of mechanisms leading to many different clinical manifestations. As a result, neurological complications of these disorders can result in significant morbidity and mortality. Advances in the diagnosis, laboratory investigations, and management of these conditions has placed an increasing burden on the practicing clinician to correctly assess and treat these patients. This volume of the Handbook of the Systemic Autoimmune Diseases will help the clinician and researcher better understand the current concepts of nervous system involvement from systemic autoimmune disorders, recognize current approaches to diagnosis and treatment, current controversies, and areas that need significant research efforts. It is believed that providing this approach in a single volume will facilitate its use as a frequent resource for all those caring for or investigating these patients and their diseases. Each chapter follows a consistent outline to provide the reader with important and useful information in an easy-to-navigate manner and is also liberally referenced to provide more primary source material for further consideration. The standardized approach to each chapter will allow the reader to be able to compare and contrast more efficiently the neurological complications of the autoimmune diseases discussed.

Is your body struggling to cooperate with you on the best of days, but you have never been able to figure out why? Do you have inflammation problems or gut sensitivities? Do you commonly suffer from anxiety? What about depression? If so, then keep reading... Within each and every one of us, there is a major nerve. This nerve runs throughout most of the body, from one end to the other, and is able to interact with all of your vital organs. From your heart to your lungs to your stomach and even your colon, one particular nerve controls it all. This nerve is your vagus nerve. Known as the vagabond nerve, this nerve is able to go throughout your entire torso, making sure that your body is functioning as it should be to ensure that, at the end of the day, your body is able to keep itself alive. It regulates your heart rate. It controls your breathing and blood pressure. It controls your digestion. It even controls how you interact with the world around you. This particular nerve also controls your ability to respond to trauma and stress. It is what is able to help you shift from calm to having the energy to protect yourself back to calm again in a relatively short period of time. However, sometimes, this nerve does not function properly. Instead of functioning like a normal nerve, it malfunctions-it causes more problems than it solves, and when that happens, your body begins to suffer for it. When the vagus nerve is lacking in tone, meaning that it is not as healthy as it could be, it begins to inflict all sorts of other problems onto you. It can lead to you struggling with anxiety or depression. It can give you brain fog or cause your digestive system to be all out of sorts. It can lead you to have autoimmune issues or even lead to inflammation throughout your body. This one tiny nerve is able to control all of that and more. However, just because it is malfunctioning does not mean that you are doomed. You can begin to train it to function properly, and this book is here to teach you how. Within this book, you will find: How the vagus nerve is able to control the vast majority of your body's vital functions without you ever being aware of it The ways in which your body is able to make use of its autonomic nervous system-the nervous system that controls your involuntary actions The healing power of the vagus nerve and how you can make it your own The power of vagal tone and how to ensure that your own nerve is functioning properly How the vagus nerve determines your trauma response The vagus nerve's involvement in the gut, inflammation, autoimmune disorders, anxiety, and more Several exercises that you can make use of to help tone your own vagus nerve to get it working for you AND MORE! Do not hesitate-if you are tired of living with these chronic issues that have been plaguing you, then take control now. Book Description :

Zinc has an important role in human physiology due to its involvement in numerous enzymatic reactions and its other functions as part of hormones and structural proteins. As a consequence a wide variety of symptoms may occur in different states of zinc deficiency. Several diseases of the digestive tract can lead to zinc deficiency and consequently to deficiency symptoms. This book, the proceedings of an International Falk Workshop held in Freiburg, Germany, on 27 October 1996, aims at a dialogue between basic researchers and gastroenterologists in order to improve the understanding of the role of zinc in disorders of the digestive tract. Based on lectures on the role of zinc in cell biology, the pathophysiology of zinc deficiency and consequently the possibilities of zinc substitution in these disorders is discussed. It is hoped that this book will initiate fruitful discussions and collaborations leading to a better knowledge of the role of this trace element in gastroenterology.
