Clinical and laboratory data as well as data obtained by a semiquantitative food frequency questionnaire of 55 consecutive patients diagnosed with NAFLD were compared to an age and gender-matched cohort of 88 healthy individuals by univariate analysis. The efficacy of the dietary intervention was assessed in a subgroup of 24 NAFLD patients 6 months after receiving dietary advice. BACKGROUND AND AIMS: Different dietary components other than energy load may be implicated in non-alcoholic fatty liver disease (NAFLD). The aim of this randomized controlled trial (NCT03380416) is to evaluate the efficacy of a dietary intervention targeting multiple dietary components (PORTFOLIO diet) on liver fat in T2D vs. a diet rich in monounsaturated fatty acids (MUFA diet), known to reduce liver fat.METHODS: According to a parallel group design, so far, thirty-eight overweight-obese individuals with T2D and NAFLD, aged 35-75 years, after a 3-week run-in, have been randomized to a PORTFOLIO or MUFA diet, similar for energy and macronutrients but not for fibre, low glycemic index foods, polyphenols, n-3 and n-6 polyunsaturated fatty acids u2014 higher in the PORTFOLIO. Before and after the 8-week intervention, liver fat content was evaluated by 1H-MRS and fasting blood samples were collected for measuring metabolic parameters.RESULTS: During the intervention, dietary adherence was optimal with no significant changes in body weight. Glycated hemoglobin and liver enzymes remained unchanged. Liver fat decreased significantly after the PORTFOLIO diet (8.8u00b18.0% vs. 5.0u00b14.7%; p=0.006) and less after the MUFA diet (9.5u00b19.3% vs. 8.1u00b18.0%; p=0.018), with a significant difference between the two regimens (p=0.043). The percentage variation of liver fat was -44% after the PORTFOLIO diet and -15% after the MUFA diet, with a significant difference between the treatments (p=0.031).CONCLUSIONS: The PORTFOLIO diet reduced liver fat content in T2D individuals with NAFLD more than a MUFA diet, suggesting that the former could be the regimen of choice for NAFLD treatment in T2D individuals.

The book is divided in two parts: Part I deals with nutrition support in cancer patients, including the specific role of nutrition on immunity, cancer cachexia, and the role of different substrates. Part II addresses nutrition in transplant patients. The first two chapters deal with the immunologic role of nutrition and cancer cachexia. Chapter 3 elegantly and extensively reviews the nutritional implications; its biochemistry and the role of one of the most studied amino acids in clinical practiceƑglutamine. This is followed by two chapters of nutrition support of patients with head and neck cancer and nutrition support of patients with gastrointestinal cancer. Chapters 6 and 7 review the role of total parenteral nutrition on perioperative nutritional support and cell cycle kinetics. Part Two of this book starts with the review of nutritional support in small bowel transplantation. This Section elegantly describes the process of recovery of small bowel from the ischemia and preservation, weaning from parenteral nutrition support and establishment of normal diets. In addition monitoring techniques and the nutritional complications of surgical intervention is described. Chapter 12 on liver failure and liver transplant patients addresses hepatic encephalopathy and the role of certain amino acids, nutrition assessment techniques and metabolic changes following liver transplantation. Furthermore, it offers some practical advice on how to establish nutrition support routes in these very ill patients. Nutrition support in renal transplantation, including metabolic abnormalities in renal failure, are described on Chapter 13.
