Book Description :

Rheumatic inflammatory diseases represent a steadily increasing group of disorders that have considerable social impact through affected patients' worsening quality of life and require engagement in the health field. Among these, rheumatoid arthritis stands as the most frequent pathology, and the hand is the most typical affected area. The incidence of rheumatic arthritis, as indicated by recent epidemiological studies, is bound to undergo a further significant increase in the future. There is, therefore, a need for careful diagnosis of the disease, based on clinical criteria designed by the American College of Rheumatology and Diagnostic Imaging. The latter provides an objective assessment of the extent and severity of joint involvement. Rheumatoid Hand includes 25 tables of clinical and radiological cases. The text analyses the capability of diagnostic imaging (conventional radiology, ultrasonography and magnetic resonance) to identify the early stages and the activity of disease, allowing the rheumatologist to decide on an effective therapeutic plan.

Book Description :

Diagnostic Joint Imaging in Rheumatology presents a practical and comprehensive approach that provides a robust framework for review and interpretation of joint imaging studies. This resource is organized by anatomic region affected - different from current books in the market organized by disease. This approach of addressing each individual region is more valuable to learners, who will be prompted to consult this reference after being presented with patients who have symptoms affecting specific anatomic regions. Patients present with pain/stiffness in affected joints/regions and not with established diagnoses. A high proportion of patients presenting to primary care practitioners, including internists, have symptoms based in musculoskeletal disease. An even higher proportion of patients presenting to rheumatologists have symptoms and signs of musculoskeletal disorders. These patients need to be assessed for joint diseases, and in addition to 'standard' data collection, diagnostic imaging is often an integral element of the evaluation. Trainees, internists and rheumatologists will benefit from the convenient access to this reference source that clearly delineates the scenarios in which joint imaging could aid in making a diagnosis, which imaging modalities to request, the sensitivity and specificity of the imaging test for each type of joint disease, most importantly - how to make their own assessment of the imaging, and when to request help from a musculoskeletal radiologist. Pulls together a collection of patient-derived imaging data that will significantly contribute to the development of more robust precision medicine approaches to joint health Focuses on simple imaging techniques that do not require expensive and technologically demanding techniques, making this relevant in lower resource settings world-wide Illustrates the importance of plain radiographs in the diagnosis of many rheumatologic diseases

Book Description :

Point-of care ultrasound, or ultrasound performed and interpreted by the clinical in the clinic/office or at the bedside, has been rapidly expanding in use among multiple medical specialties. Musculoskeletal ultrasound has added value to clinical care to expedite diagnoses by visualizing mechanical versus inflammatory features and correlating these findings with patient’s symptoms. The use of power Doppler to assess for subclinical inflammation or active hyperemia has been a distinguishing use of ultrasound for rheumatologists. Accurate needle guidance with ultrasound for joint and tendon procedures has allowed expansion of procedures in the clinic and has been associated with less pain and precise targeting of affected structures. Musculoskeletal ultrasound education is now offered in nearly all rheumatology fellowship training programs in the United States and is also included in residency training in other specialties (e.g. physical medicine and rehabilitation, orthopedics, podiatry, emergency medicine, and family medicine). Since the publication of the first edition of Musculoskeletal Ultrasound in Rheumatology Review, there has been tremendous growth of musculoskeletal ultrasound in the field of rheumatology as well as other musculoskeletal specialties. The expanded second edition of this practical guide provides an updated clinical review of diagnostic and interventional applications of musculoskeletal ultrasound in rheumatology. New and revised chapters focus on the use of ultrasound in the diagnosis of specific rheumatic diseases such as osteoarthritis, scleroderma, psoriatic arthritis, gout, and lupus. Each chapter covers a standardized protocol of joint images with probe placement and includes numerous examples of common ultrasound pathologies. Study tools such as key-concept overviews, lists of important studies in the field, and extensive questions for self-assessment are included throughout. Written by current experts and thought leaders in the rapidly advancing field of rheumatology ultrasound, the second edition of Musculoskeletal Ultrasound in Rheumatology Review is an essential reference for physicians and related professionals, residents, fellows, graduate students and nurses in rheumatology, imaging and radiology, sports medicine, internal medicine, and physiotherapy.

Book Description :

The introduction of novel therapeutic strategies set new goals for the patients’ outcome, which aims to achieve remission. This goal requires early diagnosis of RA and prompt efficient pharmacotherapy. The introduction of anti-citrullinated protein antibodies (ACPA) two decades ago allowed an earlier RA diagnosis. However, there are indications that ACPA positivity is still associated with higher rates of radiographic damage. As the small joints in hands and feet commonly are the first involved sites of inflammation, the role of different imaging modalities were studied regarding their diagnostic and prognostic impact for assessment of arthritis in RA. Further, ultrasound (US) and radiography were used to study the association between RA-specific antibodies and the occurrence of arthritis and joint damage in systemic lupus erythematosus (SLE). The use of US allows assessment of soft tissue like joint capsules, tendons and bursae. Used for a live scanning, it is easy to detect effusions and edema. Doppler indicates vasoproliferation were inflammation is present. Also, US seems to be more sensitive than radiography to detect minimal structural changes located at bone surfaces. We wanted to investigate whether US findings in a pre-RA stage can predict development of arthritis. Digital X-ray radiogrammetry (DXR) is a technique based on computerized analyses of standard hand radiographs to calculate peripheral bone mineral density (BMD) of the three middle metacarpal bones (DXR-BMD). In order for early treatment decisions, we aimed to study whether changes in DXR-BMD loss after 3 months can predict radiographic damage in early RA. In conclusion, the studies showed that ACPA-positivity is still associated with a higher risk of radiographic damage regardless of early treatment decisions. Therefore, close radiographic monitoring and readiness to intensive treatment is warranted in ACPA-positive patients. This thesis also shows that erosions detected by US in ACPA-positive patients with arthralgia predict development of clinical arthritis. Also, the magnitude of DXR-BMD loss helps identify patients at higher risk for future radiographic damage, and may therefore help to improve early treatment decisions. Finally, US and radiography confirm a higher rate of arthritis and erosions also in SLE patients who are positive for RA-specific antibodies.

Book Description :

In the present monograph, we offer current insights into polymyalgia rheumatica and giant cell arthritis. Both diseases are typical for advanced age, and their incidences increase with aging. Both diseases are a center point of interest not only for rheu- tologists, gerontologists, ophthalmologists or neurologists, but also for general prac- tioners. Early diagnosis and rapid treatment, mainly with glucocorticoids can save one of the most precious senses-vision. Damage to other organs (heart, aorta, co- nary arteries, liver, lungs, kidneys), which are supplied by the arteries affected by ischemic syndrome in the setting of giant cell arthritis, has serious consequences as well. Late diagnosis of giant cell arthritis can have fatal consequences for affected patients. It is a matter of fact that the human population is aging. Therefore, more attention has to be paid not only to diagnosis, clinical course and treatment of rheumatic d- eases in elderly, but also to their genetic, immunologic, endocrinologic, chronobiologic mechanisms, and state-of-the-art diagnostic modalities. I am convinced that the int- disciplinary research of the diseases will allow us to diagnose and treat the rheumatic diseases even faster and more effectively in the future.
