Background: Digital health technology has been developed to support diabetes self-management, but its effectiveness could depend on patient engagement. We examined the relationship between novel user utility score (UUS) developed to measure patient engagement and glycemic control among a policyholders with type 2 diabetes. Methods: In 72 participants, we analyzed the association of health outcomes over 6 months with UUS for the first 3 months through a sub-analysis of results from a 6-month randomized controlled trial of a tailored mobile coaching (TMC) system. The UUS was calculated as the sum of scores for four major core components (range 0u20138): frequency of self-monitoring blood glucose testing, dietary and exercise records, and message reading rate. Results: Participants were divided into two groups: UUS:0u20134 (n = 39) and UUS:5u20138 (n = 33). At 3 months, the UUS:0u20134 and UUS:5u20138 groups had decreases in HbA1c of 0.39 u00b1 0.73% and 0.99 u00b1 1.43%, respectively (P = 0.034). Changes in HbA1c over 6 months persisted in both groups (-0.35 u00b1 1.08 vs. -0.98 u00b1 1.11; P = 0.017). Changes in HbA1c at 3 and 6 months were significantly associated with UUS. The purpose of developing mobile applications for diabetes is generally to: (1) provide enhanced access to timely information for patients, health care professionals, and researchers; (2) facilitate remote monitoring and diagnosis of patients, often based on information delivered by wearable devices; (3) provide decision support to assist patients in selecting treatment; or (4) deliver timely recommendations for treatment to increase adherence to prescribed therapy. There is a perception that mobile applications can provide meaningful clinical benefits, however, there is only sparse convincing evidence to support this belief at the present time. Compounding this problem is the short life span of digital software, such that if a traditional type of randomized controlled trial is conducted on a product, by the time the study has been designed, approved by an IRB, conducted, and analyzed, the product might have significantly changed to a next generation system. Because of great interest in establishing what are the potential benefits, metrics of success, and appropriate components of mobile applications for diabetes, Diabetes Technology Society and William Sansum Diabetes Center launched the Digital Diabetes Congress, March 7-8, 2017, in San Francisco. This report contains summaries of the meeting’s 12 sessions. Each summary was written by the session’s moderator who helped develop the session prior to the event and keep it on track during the event.
