Attention-deficit and hyperactivity disorder (ADHD) is one of the most frequently diagnosed disorders. Nevertheless, phenotypes of ADHD are heterogenous, and remain vaguely defined. For instance, current cognitive test procedures are often inconclusive on the characteristics of ADHD and frequent comorbidities are mostly neglected. This thesis is composed of three parts: The first part describes existing approaches (e.g., theories, ADHD subtypes, comorbidities), as well as empirical findings from current test procedures (e.g., neurocognitive tasks) for ADHD across eight cognitive domains. It will be shown that the findings from these reviews point towards a spectrum of ADHD and comorbidities that often seem beyond the scope of existing studies, because of the nature of the administered test procedure, the type of applied analysis, as well as the pre-categorization of participants. The third part therefore introduces a proposal for measuring a spectrum of not only ADHD, but also comorbidities in a set of test procedures susceptible to identified clinical characteristics. To support these proposals, the second part describes the results from the application of the Ratcliff Diffusion Model to the neurocognitive test performances of an existent clinical dataset (e.g., the MTA study). It will be illustrated that significant differences between controls and ADHD children become apparent, when accounting for gender and medical treatment. Specifically, unmedicated ADHD children seem to take less time for cue retrieval and motor output (smaller Ter), accumulate information at a slower rate (smaller v), and show higher variability in response times. ADHD boys are characterized by a small Ter when interstimulus interval is short, whereas ADHD girls are characterized by an overly conservative decision criterion (large a). Moreover, stimulants not only increase drift rates as suggested by previous studies, but also reduce the gender-specific ADHD characteristics (e.g., smaller Ter for ADHD boys and larger a for ADHD girls). Eventually, the preliminary results, together with the literature reviews, suggest that estimated RDM parameters can be used to classify ADHD phenotypes. Understanding the cognitive characteristics of various ADHD phenotypes (incl. comorbidities) is important to identify the most effective medical and/or behavior interventions.

In this book, Andreas Heinz proposes a computational and dimensional approach to understanding and classifying mental disorders: modeling key learning and decision-making mechanisms across different mental disorders. Such an approach focuses on the malleability and diversity of human behavior and its biological underpinnings. Heinz explains basic learning mechanisms and their effects on human behavior, focusing not on single disorders but on how such mechanisms work in a multitude of mental states. For example, he traces alterations in dopamine-reinforcement learning in psychotic, affective, and addictive disorders. He investigates to what extent these basic dimensions of mental disorders can account for such syndromes as craving and loss of control in addiction, positive and negative mood states in affective disorders, and the altered experience of self and world associated with psychotic states. There is a growing appreciation that many psychiatric (and neurological) conditions can be understood as functional disconnection syndromes – as reflected in aberrant functional integration and synaptic connectivity. This Research Topic considers recent advances in understanding psychopathology in terms of aberrant effective connectivity – as measured noninvasively using functional magnetic resonance imaging (fMRI). Recently, there has been increasing interest in inferring directed connectivity (effective connectivity) from fMRI data. Effective connectivity refers to the influence that one neural system exerts over another and quantifies the directed coupling among brain regions – and how they change with pathophysiology. Compared to functional connectivity, effective connectivity allows one to understand how brain regions interact with each other in terms of context sensitive changes and directed coupling – and therefore may provide mechanistic insights into the neural basis of psychopathology. Established models of effective connectivity include psychophysiological interaction (PPI), structural equation modeling (SEM) and dynamic causal modelling (DCM). DCM is unique because it explicitly models the interaction among brain regions in terms of latent neuronal activity. Moreover, recent advances in DCM such as stochastic and spectral DCM, make it possible to characterize the interaction between different brain regions both at rest and during a cognitive task.
