Dural arteriovenous fistula (dAVF) is a rare disease characterized by anomalous communication between dural arteries and venous sinus and/or cortical veins. dAVF is responsible for 10-15% of all intracranial vascular malformations, and affects mainly the group between 50-60 years old. OBJECTIVE: Report and illustrate two clinical cases of dAVF: one u201cbenignu201d and other u201cmalignantu201d. METHOD: Case reports. Case1 (benign dAVF): Female, 27yo, with headache for 3 months, and cranial MRI showing a right transverse-sigmoid sinus thrombosis. She was successfully treated with Warfarin 5mg/day. Three months later, started with tinnitus and new MRI showed an early filling of the right dural transverse sinus. In the cerebral digital subtraction angiography (DSA), a dAVF was found between branches of the external carotid artery and the right transverse sinus (BordenI; CognardIIa). After endovascular embolization with Onyxu00ae, she completely improved her symptoms. Case2 (malignant dAVF): Male, 56yo, with headache for 5 days, vomit and dizziness. On physical examination, had ataxia and bilateral papilledema. Cranial MRI showed multiple anomalous vascular structures with tubular enhancement in left cerebellar hemisphere. He underwent cerebral DSA, which confirmed dAVF with venous cortical reflux and venous ectasia (BordenIII; CognardIV). During hospitalization, his headache worsened and cranial CT revealed acute hemorrhage in the left cerebellar hemisphere with compression of the fourth ventricle and hydrocephalus, requiring external ventricular shunt. He underwent dAVF embolization with Onyxu00ae and had complete improvement of symptoms. CONCLUSION: The knowledge of different dAVF presentation (benign and malignant) is of fundamental importance.

Background and aims:Dural arteriovenous fistulas (DAVF) are a rare type of vascular malformation in which there is a pathological communication between dural arteries and veins or venous sinus. Clinical presentation and severity depend on drainage type and location.Methods:Retrospective, descriptive study including patients seen in our centre during 10 years. DAVF were classified using Cognard scale. Epidemiology, location, drainage, clinical presentation, trearment and prognosis were analyzed. Results:34 patients were diagnosed with DAVF during this period. Median age was 61 years old and most of them were male (73.5%). In 23 patients (67.6%) the etiology was unknown. Clinical presentation was severe (hemorrhage, seizures, focal deficit) in 24 patients (70.7%), rest of them had minor symptoms as thrill or headache, or were asymptomatic. 6 patients (17.6%) first contact was as a stroke code. 23 of the 24 patients presenting with severe symptoms were Cognard >/=IIb. 24 patients (70.6%) received treatment, of which 16 (66.7%) was surgical. Complete obliteration of DAVF was achieved in 17 (70.8%). After discharge, 7 patients (20.6%) presented mRankin >2, all of them had severe clinical presentation and Cognard >/= IIb. Conclusions: Worse prognosis was seen in patients who have direct cortical vein drainage and/or agressive clinical presentation. Surgical treatment was the most frecuently used in our centre, getting good results in the majority of cases.

Here we present a case of a 55 year old lady who presented to the hyperacute stroke unit with ageneralized seizure following a two day history of headache and right sided homonymoushemianopia. An urgent CT brain showed a left occipital intracranial haemorrhage, however theclinical presentation and location of the haemorrhage raised suspicion of a cerebralvenousthrombosis (CVT). CT cerebral angiogram demonstrated a likely dAVF. Transfer was arrangedto the regional neurosurgical centre where a Type IIb dAVF of the left transverse sigmoid junctionwith thrombosis was confirmed angiographically. The patient underwent successful endovascularembolization.A dural arteriovenous fistula (dAVF) is an abnormal direct connection between a dural artery and adural venous sinus, cortical vein or meningeal vein. These are graded by venous drainage in theCognard classification. dAVF of grade IIb-V carry an annual mortality of u224810% and a haemorrhagerisk of u22488% (1).This case illustrates a clinically relevant mimic to a cerebral venous thrombosis. The management ofthese conditions is very different, CVT requiring anticoagulation and dAVF commonly requiringendovascular intervention and avoidance of anticoagulation, making an awareness of this possiblemimic important for stroke physicians.

Background - Dural arteriovenous fistulae (DAVF) are an uncommon complication of cerebral venous thrombosis (CVT). It is unclear why some patients develop dAVF after CVT, the timeframe for development nor the optimal treatment or outcome. We reviewed existing literature to identify demographics, complications and management of DAVFMethods u2013 A systematic review was performed using multiple electronic databases with PRISMA guidance. Published studies reporting DAVF at, or after CVT diagnosis and unpublished local service data were included. Manuscripts were independently screened for inclusion by two reviewers and data analysis undertaken descriptively, using summary statistics. Results u2013 45 patients were included from 37 manuscripts and local service data. 62.5% cases were male. The median age of CVT diagnosis was 54, being higher for male than female (56.5 vs 47.0 years, p = 0.032). Median time of onset of DAVF after CVT was 12 months (range 1 u2013 132 months, excluding 23 cases with concomitant DAVF and CVT diagnoses). Treatment was endovascular in 55.1% cases, surgical in 18.4%, and conservative in 20.4% with radiological resolution reported in 38.8% cases and improvement in clinical features in 51%. No radiological or clinical outcomes were reported in 44.9% and 38.8% cases respectively.Conclusion In contrast to the female preponderance both for CVT and dAVF the male preponderance for dAVF after CVT was striking, as was the relatively older ages reported. Reporting of outcomes was poor, highlighting a need for disease registries with standardised reporting to better understand risk factors for developing an uncommon complication of an uncommon disease, and its treatment.
