For many years now I have devoted much of my time to lecturing and writing on the subject of cardiovascular disease (CVD). In this book I have outlined the development of my approach to this problem. I must admit that the details of my theories have undergone continuous modification as a result of the lessons learned in treating a large number of patients, but the basic message has remained the same. I believe that the delay in the utilization oftestosterone treatment for CVD has arisen from a failure by specialists in endocrinology, biochemistry, physi ology, and cardiology to understand each other's point of view and therefore to effectively coordinate their clinical efforts. This is like four people starting to climb the various faces of a pyramid, unaware of each other's presence until they reach the apex. It is hoped that bringing specialists in these different disciplines together at "summit meetings" will help them discover the true nature of this disease, the cardiovascular specialist understanding the underlying lack of anabolic steroids, and the other three grasping the way in which treatment with these compounds can effectively counteract the metabolic disturbance which is the cause of CVD. Even apart of the language barrier, writing this book has by no means been an easy task. Please consider my situation; one cannot be a cardiologist, endocrinologist, biologist, biochemist, and physiologist all in one person. This fact may open me to criticism.

Cardiovascular diseases (CVD) are increasing in epidemic proportions in developing countries. CVD already accounts for almost 10 percent of the developing world's burden of disease and is likely to become the developing world's leading cause of death. There is reason for hope, however, given that huge potential exists for applying R&D to control this emerging epidemic--both in creating powerful new interventions such as vaccines and dietary supplements and in guiding behavior. In addition, a considerable body of evidence suggests that current risk-factor prevention programs and low-cost case management of CVD offer feasible, cost-effective ways to reduce CVD mortality and disability in developing country populations. Large-scale CVD control efforts are lacking, however, and thus governments and individuals are left to make choices about health and health care services without the benefit of appropriate knowledge. This report was designed to promote a policy dialogue on CVD based on informed knowledge of R&D opportunities that offer effective, affordable, and widely applicable responses in developing countries. This book describes a wide range of interventional procedures currently employed in the treatment of cardiac diseases in women with the aim of enabling not only interventional cardiologists but also cardiologists and cardiac surgeons to refine their approach to female patients and thus improve outcomes. The book reflects the increasing awareness of gender differences with regard to coronary artery disease and structural valve heart disease. Examples include myocardial infarction, which carries a worse prognosis in women in part because they suffer from this disease mostly in the 7th decade of life, present more frequently with hemodynamic instability and later from symptom onset, and have fragile, small and tortuous coronary arteries that are more technically demanding in the context of primary percutaneous coronary intervention. Another example is symptomatic aortic stenosis, which carries a better prognosis in women who undergo trans-catheter aortic valve implantation partially due to a better annulus-prosthesis match, which leads to less residual paravalvular regurgitation, better left ventricular function and lower aggregation of comorbidities. Further, in women left ventricular response to aortic stenosis is typified by muscle hypertrophy with a small ventricular cavity. These differences are reflected in the clinical manifestations and their impact on diagnosis, treatment, and choice of tools and devices for use in interventional procedures. By explaining the at times subtle variances in disease and treatment techniques for female and male patients, this book will help practitioners select the most appropriate procedures and materials in a range of settings.
