More specifically, different sub-specialties of omics biotechnology including: genomic, proteomic, glycomic, lipidomic, metabolomics, metalomic, and microRNA biomarkers are described to accomplish early and personalized theranostics of chronic illnesses with currently limited therapeutic options and success. Recent advances in nanotechnology and novel biomarkers to accomplish personalized theranostics of chronic diseases including, but not limited to: cancer, fetal alcohol syndrome, Alzheimer''s disease, Parkinson''s disease, vascular dementia, amyotrophic lateral sclerosis, and multiple sclerosis. Usually, nanoparticles (NPs) are trapped in the reticuloendothelial system (RES) comprising the lungs, liver, and spleen. To prevent their entry into the RES system and enhance drug targeting, stealth property of polyethylene glycol (PEG) as a covalent linker is utilized. However, pegylation may reduce the biological activity of the antibody or other labile molecule attached to NPs. Therefore, positively-charged chitosans and negatively charged alginates have been developed. Liposomal encapsulated Doxorubicin (Adriamycin) was approved in the early seventies for the treatment of breast cancer. In addition, enhanced permeability and retention (EPR) property of newly synthesized micro vessels of a hyper proliferating tumor is utilized to selectively invade the cancer stem cells is intricately described. Various noninvasive in-vivo imaging technologies including computerized tomography (CT), magnetic resonance imaging (MRI) along with magnetic-resonance spectroscopic (MRS) capability, single photon emission tomography (SPECT), positron emission tomography (PET), ultra sound alone or as hybrid images to obtain precise 5D anatomical landmarks, regional physiology, and biochemistry of the human body organs at cellular, molecular, and genetic level in health and disease are described. The book also describes in-vitro methods to acquire images of tissues and cells in culture employing digital infrared imaging, digital fluorescence microscopic, confocal microscopic, and fluorescence resonance transfer (FRET) resonance imaging for detecting apoptosis and/or necrosis at the cellular, molecular and genetic level. This timely-released manuscript will attract basic scientists, researchers, and physicians interested in further developing the emerging concept of personalized medicine (PM), and will serve as a textbook for Masters, M.D., and Ph. D students, and a reference book for doctors, professors, basic researchers, scientists, and general public. Positron emission tomography (PET) is a non-invasive imaging method that renders three-dimensional images of tissue that selectively has taken up a radiolabelled organic compound, referred to as a radiotracer. This excellent technique provides clinicians with a tool to monitor disease progression and to evaluate how the patient respond to treatment. The by far most widely employed radiotracer in PET is called 2-deoxy-2-[18F]fluoro-D-glucose ([18F]FDG), which is often referred to as the golden standard in PET. From a molecular perspective, [18F]FDG is an analogue of glucose where a hydroxyl group has been replaced with a radioactive fluorine atom (18F). It is well known that covalent attachment of carbohydrates (i.e., glycosylation) to biomolecules tend to improve their properties in the body, in terms of; improved pharmacokinetics, increased metabolic stability and faster clearance from blood and other non-specific tissue. It is therefore natural to pursuit the development of a [18F]fluoroglycosylation method where [18F]FDG is chemically conjugated to a ligand with high affinity for a given biological target (e.g., tumors or disease-associated protein aggregates). This thesis describes a novel [18F]fluoroglycosylation method that in a simple and general manner facilitate the conjugation of [18F]FDG to biological ligands using click chemistry. The utility of the developed [18F]fluoroglycosylation method is demonstrated by radiolabelling of curcumin, thus forming a tracer that may be employed for diagnosis of Alzheimer’s disease. Moreover, a set of oligothiophenes were fluoroglycosylated for potential diagnosis of Alzheimer’s disease but also for other much rarer protein misfolding diseases (e.g., Creutzfeldt-Jakob disease and systemic amyloidosis). In addition, the synthesis of a series of 19F-fluoroglycosylated porphyrins is described which exhibited promising properties not only to detect but also to treat melanoma cancer. Lastly, the synthesis of a set of 19F-fluorinated E-stilbenes, structurally based on the antioxidant resveratrol is presented. The E-stilbenes were evaluated for their capacity to spectrally distinguish between native and protofibrillar transthyretin in the pursuit of finding diagnostic markers for the rare but severe disease, transthyretin amyloidosis.

Alzheimer's Disease (AD) is the most prevalent neurodegenerative disease, currently affecting 5 million in the US and 25 million population worldwide. Unfortunately, there is no treatment for AD, and its accurate diagnosis requires a detailed post-mortem examination of the brain. The formation of extracellular amyloid plaques containing the amyloid [beta] (A[beta]) peptide is one of the pathological hallmarks in the brains of Alzheimer's patients. In addition, numerous studies show that the metal ions can interact with the A[beta] peptides and affect the aggregation process dramatically, especially for copper ions that can stabilize the A[beta] oligomers, leading to high neurotoxicity.In this regard, we developed a series of novel bifunctional molecules that have high binding affinity with various A[beta]42 species and strong metal-binding ligands to modulate the interaction of copper ions with A[beta]42 species to alleviate the Cu2+-induced A[beta]42 cytotoxicity. Different A[beta] binding fragments, bis-styryl stilbene and benzofuran derivatives, were synthesized successfully and modified by adding strong metal-binding ligand through the Mannich reaction. Several in vitro assays were performed to evaluate the ability to quench ROS and attenuate the cytotoxicity of Cu2+-induced A[beta] species. Moreover, immunostaining studies were also employed to see the binding specificity of the compounds. The fluorescence images show that most of the bifunctional compounds can bind to the amyloid plaques specifically. More interestingly, some of the compounds show high binding specificity of soluble A[beta] oligomers, confirming by the oligomer-specific antibody labeling.Furthermore, the novel bifunctional compounds were also evaluated as PET imaging agents for AD. Taking advantage of the high binding affinity with Cu2+, the developed bifunctional compounds were preloaded with 64Cu, and these 64Cu complexes were evaluated to be potential PET imaging agents for Alzheimer's Disease. According to the in vitro results, several compounds were selected for biodistribution studies to characterize further the imaging properties in vivo. Overall, these studies lend promise to our future studies aimed at developing improved bifunctional compounds as novel theranostic agents, given the dearth of treatment and diagnostic tools for AD.
