3D-printing in clinical orthopaedics has advanced in the areas of preoperative planning, surgical guides, rehabilitation devices, surgical simulation, and prototype prostheses. The role of 3D-printing clinical implants has not been explored because of limitations in biocompatible materials, costs, and printer resolution. In this study, we examine the design method, time-to-print, cost analysis, and feasibility of 3D-printing orthopaedic implants. While widely available in developed countries, surgical plates and screws are not always available in remote and developing areas of the world. 3D-printing offers the potential to correct this supply issue in orthopaedic care.Using open-source software and inexpensive materials, we describe a method that successfully created a database of 3D models of 8-hole 1/3 tubular plates and cortical screws. Plates were printed with 1mm, 2mm, and 3mm thicknesses. The 1mm plates printed in 9min 47sec (cost = $0.02), the 2mm plates in 14min 3sec (cost = $0.05), and the 3mm plates in 19min 11sec (cost = $0.08). All 60 screw designs had print times from 6-9 minutes, costing $0.01/screw after rounding up to the nearest cent. Overall, printing the plate and screws for an orthopaedic procedure costs less than $1.00 and takes 1-1.5 hours.

One of the most promising advances raised by the current computer age is performing research "in silico," which means computer-assisted. The objective of this chapter is firstly to evaluate if a 3D in-silico model of an oncological patient could be used to make a 3D-printed prototype in real scale, discriminating precisely healthy tissues, tumoral tissues and oncological margins. Secondly, the objective is to evaluate if this prototype could be representative enough to allow testing osteotomies under navigated guidance based on images. A tumor resection for a patient with diagnosed metaphyseal osteosarcoma of the proximal tibia was transferred into a rapid prototyping model, fabricated using 3D printing and representing different structures in different colours. The planned osteotomy was executed using Stryker Navigator to guide the cutting saw and the prototype was opened to verify the precision of the performed osteotomy. Both osteotomy planes showed successful correspondence with the safe margin, with a maximum error of 1 mm. The application of these techniques in general orthopedics would help to reduce the incidence of unforeseen intraoperative failures, contributing to obtain predictable surgical procedures. This would implement a new way of performing development, research and training in orthopedics and traumatology by in-silico technology.
