The new edition of 50 Challenging Spinal Pain Syndrome Cases brings together a total of 100 individual studies ranging from the common and comparatively straightforward to the more complex. Content is presented in a clear layout with details of case history, aetiology, physical examination, imaging and, when necessary, laboratory test results of patients, diagnosis, treatment and results, key points and suggestions for further reading. Comparisons between imaging and similar anatomical and or histopathological findings are used. Questions of anatomy are clarified and the cases are comprehensively illustrated with radiographs and scans to enable the reader to see the most likely nature of the pathology causing the patient's spinal pain syndrome. The problem-solving approach keeps the emphasis on the practical and corrects some common myths about spinal pain treatment. Taken together, these cases make up a book, which students, practitioners and everyone interested in the spine will want to own and constantly refer to. new cases, particularly in the cervical and thoracic spine sections detailed section on possible anatomical pain generators colour photomicrographs

Book Description :

Low back pain is described as a very common condition that tends to affect about 70% of the population at some point in time with varying degrees of symptom severity. Although definitions vary, sciatic pain is generally defined as back-related pain radiating to the leg (normally below the knee and into the foot and toes) and is one of the commonest variations of low back pain. Patients with sciatica typically experience a more persistent and severe type of pain, a less favorable outcome, consume more healthcare resources and have more prolonged disability and absence from work than those with low back pain alone. Managing Sciatica and Radiculopathies in Primary Care Practice provides a comprehensive, up-to-date overview of the subject and key information for primary care practitioners about low back pain in patients, including definitions and causes, current management approaches, diagnostic and treatment algorithms, as well as clinical practice guidelines.

Background and Aims:Complex regional pain syndrome is currentlyapoorly understood disease. Symptoms are usually very intense and profoundly affect the quality of lifeofpeople who suffer from the disease. We present the case of a woman withComplex regional pain syndrome in externalmalleolus due to fall skiing.Methods:52 year old female presented to the pain department with severe external malleolus pain and stiffness, persisting for more than 8 months after falling in the snow. The patient met the criteria for a probable diagnosis of complex regional pain syndrome (CRPS) ankle. She had already been treated with corticoids and we decided to discontinue the treatment as no response was found. We prescribed gabapentin and tramadol with almost no response. After this treatment we undergo a superficial peroneus block with 100 % improvement that lasted 3 weeks, therefore we decided to apply radiofrequency to that nerve during 6 minutes and 45 V.ResultsAfter the radiofrequency, we applied 3 milliliters of levobupivacaine plus 4 milligrams of dexamethasone. The patient had an improvement of 100 % that lasted at least during 6 months. She could come back to work and we follow her up every 3 months. Conclusions:TheComplex regional pain syndrome is a pain treatment challenge. We must have open minds and offer our patient the best option according to their illness.

Book Description :

Endoscopy of the spinal canal – epiduroscopy (EDS) – has proven to be a safe, efficient and future-oriented interventional endoscopic procedure for everyday clinical use in diagnosing and managing pain syndromes. Epiduroscopy can be used in the sacral, lumbar, thoracic and even cervical regions of the spine to identify pathological structures, carry out tissue biopsies and perform epidural pain provocation tests to assess the pain relevance of visualized anomalies, making it an excellent diagnostic tool. Spinal endoscopy allows targeted epidural analgesic pharmacologic therapy for affected nerve roots or other painful regions in the epidural space. Treatment options provided by epiduroscopy include laser-assisted adhesiolysis or resection of pain-generating fibrosis, catheter placement, as well as support with other invasive procedures for pain relief. Professional EDS management enhances a multimodal philosophy and opens up new treatment strategies for patients. If used early on, it can control pain well before chronicity sets in.

This book addresses all pain imaging aspects related to both the central nervous system and the body (thorax, abdomen and pelvis), thus updating the international literature on the topic. By adopting a clinical-radiological approach and offering a comprehensive differential diagnosis for a number of painful syndromes (many of which can mimic one other), the work aims to support and enhance the diagnostic management of these patients, suggesting the most appropriate diagnostic algorithm. The book is divided into separate sections for each anatomical macro-area, and the chapters cover the respective topics from both clinical and radiological perspectives. Further, the book includes extensive electronic supplementary material. As such, it offers an invaluable tool for radiologists, neuroradiologists and clinicians working in internal medicine, surgery and neurology, and could also be used in residency programs for these groups.
